Phase 3 Results Show Survival Benefit
The DUBLIN-3 study evaluated plinabulin in combination with docetaxel in second- and third-line EGFR (search) wild-type NSCLC patients. In the mechanism-targeted non-squamous NSCLC population (n=332), the combination achieved an overall survival hazard ratio of 0.72 (p=0.0078), representing a median overall survival improvement of 2.5 months versus docetaxel alone. The treatment also doubled the 2-year and 3-year survival rates while maintaining a favorable safety profile, reducing grade 4 neutropenia from over 30% to 5% (p<0.0001).
Based on these findings and discussions with the U.S. FDA, BeyondSpring plans to initiate the global Phase 3 DUBLIN-4 confirmatory study. This trial will focus on a mechanism-enriched patient population of EGFR (search) wild-type non-squamous NSCLC patients who have progressed on prior PD-1 (search)/L1 inhibitors, with overall survival as the primary endpoint.
Addressing Checkpoint Inhibitor Resistance

The combination demonstrated a median progression-free survival of 7.0 months, with a disease control rate of 85% and overall response rate of 18.2%. The median overall survival was not reached, with a 24-month overall survival rate of 66%.
Additionally, a Phase 1 study at MD Anderson Cancer Center across eight tumor types resistant to checkpoint inhibitors showed a disease control rate of 54% and overall response rate of 23% when plinabulin was combined with PD-1 (search) inhibitor and radiation therapy. The study provided mechanistic evidence of dendritic cell maturation and immune activation, supporting plinabulin's proposed immune-priming mechanism.

Plinabulin functions as a dendritic cell maturation agent, supporting both anti-cancer activity and immune modulation.
