The breakthrough study, published in the Proceedings of the National Academy of Sciences, demonstrates how genetically modified bacteria can serve as living drug factories to deliver therapeutic compounds directly through oral administration.
Novel Probiotic Platform Addresses Treatment Compliance Issues
Fear of needles, injection-associated infections, and pain frequently cause patients with chronic conditions to skip doses, creating a critical need for alternative delivery methods. "People don't like to have injections for the rest of their lives," said Dr. Christine Beeton, professor of integrative physiology at Baylor and co-corresponding author of the study.
The research team, led by Beeton in collaboration with Dr. Robert A. Britton, professor of molecular virology and microbiology at Baylor, bioengineered the probiotic Lactobacillus reuteri to secrete peptide ShK-235 (search), a therapeutic compound derived from sea anemone toxin (search). Previous work from Beeton's lab had shown this peptide effectively and safely reduces disease severity in rat models of rheumatoid arthritis (search) and patients with plaque psoriasis (search), but required repeated injections with low oral efficacy.
Strategic Choice of L. reuteri as Delivery Vehicle
The researchers selected L. reuteri for several key advantages. These bacteria are indigenous to human and animal guts and belong to the lactic acid bacteria groups long used as cell factories in the food industry. The U.S. Food and Drug Administration recognizes L. reuteri as safe, with an excellent safety profile demonstrated in infants, children, adults, and even immunosuppressed populations.
Critically, L. reuteri does not remain in the gut permanently, as the bacteria are removed during regular renewal of the gut's inner surface layer. "This opens the possibility for regulating treatment administration," Beeton explained.
Dramatic Disease Reduction in Animal Models
The bioengineered strain, designated LrS235, demonstrated remarkable therapeutic efficacy in animal models of rheumatoid arthritis (search). Daily delivery of these peptide-secreting bacteria dramatically reduced clinical signs of disease, including joint inflammation, cartilage destruction, and bone damage.
The research team tracked both the LrS235 bacteria and the ShK-235 (search) peptide it secretes within the animal model. After feeding rats live LrS235 that release ShK-235, they successfully detected the therapeutic peptide in blood circulation, confirming systemic delivery through oral administration.
Patient Convenience and Clinical Potential
The oral delivery system offers significant practical advantages over traditional injection-based therapies. "These bacteria could be stored in capsules that can be kept on the kitchen counter," Beeton noted. "A patient could take the capsules when on vacation without the need of refrigeration or carrying needles and continue treatment without the inconvenience of daily injections."
This convenience factor could dramatically improve patient compliance, addressing one of the major challenges in managing chronic inflammatory conditions that require long-term treatment.
Broader Applications for Chronic Disease Management
The findings provide an alternative delivery strategy for peptide-based drugs and suggest that such techniques and principles can be applied to a broader range of drugs and treatment of chronic inflammatory diseases. The probiotic-based platform could potentially revolutionize how various therapeutic compounds are delivered, particularly those that currently require injection or have poor oral bioavailability.
