Canada's Drug Agency (search) (CDA-AMC) has issued a positive reimbursement recommendation for Bristol Myers Squibb (search)'s OPDIVO (nivolumab) in combination with YERVOY (ipilimumab) for the first-line treatment of adult patients with unresectable or metastatic microsatellite instability-high (MSI-H) or mismatch repair deficient (dMMR) colorectal cancer (search) who meet certain criteria.
The pCODR Expert Review Committee (pERC) based the recommendation on results from the pivotal Phase 3 CheckMate-8HW trial involving 303 patients, which demonstrated that the OPDIVO plus YERVOY combination delivered a statistically significant and clinically meaningful improvement in progression-free survival compared with standard chemotherapy. After a median follow-up of 42 months, the median progression-free survival was 54.1 months with the immunotherapy combination versus 5.9 months with chemotherapy (HR 0.21; 95% CI: 0.14–0.31).
Ontario Leads Implementation Through FAST Program
Following the CDA recommendation, Ontario has become the first jurisdiction in Canada to reimburse OPDIVO plus YERVOY for eligible patients under the province's newly launched Funding Accelerated for Specific Treatments (FAST) program. The funding applies to first-line treatment of adult patients with locally advanced unresectable or metastatic MSI-H or dMMR disease, excluding individuals with significant autoimmune disease or a history of interstitial lung disease or pneumonitis.
This decision represents a significant milestone as OPDIVO plus YERVOY is among the first therapies added to Ontario's New Drug Funding Program through the FAST program framework.

The positive recommendation addresses an important unmet medical need for patients with MSI-H or dMMR metastatic colorectal cancer (search), a distinct molecular subtype that has had limited treatment options in the first-line setting.
