Causeway Therapeutics (搜索) has announced positive topline data from its Phase 2 clinical trial evaluating TenoMiR® (CWT-001), a proprietary microRNA-29a (搜索) (miR-29a (搜索)) mimic, in patients with lateral epicondylitis (搜索), commonly known as tennis elbow (搜索). The Glasgow-based clinical-stage biotechnology company reported that while the full analysis population did not achieve the primary pain endpoint, patients who received appropriately targeted injections showed statistically significant improvements across multiple endpoints.
Study Design and Patient Population
The Phase 2 trial was a multi-site, randomized, double-blind, placebo-controlled, parallel-group study conducted across the USA and UK, evaluating the efficacy, safety and pharmacokinetics of TenoMiR administered via intratendinous injection in 123 subjects with lateral epicondylitis (搜索). Two dose levels of TenoMiR given as a single injection were tested, with subjects followed for 90 days.
A critical finding emerged regarding injection precision: of the 123 subjects enrolled, 90 patients (73%) received the correctly delivered dose across both TenoMiR and placebo groups. The remaining patients did not receive the injection into the optimal part of the tendon, which led to the intention-to-treat analysis not meeting prespecified efficacy criteria.
Efficacy Results in Appropriately Dosed Patients
Among the 90 patients who received the target dose of TenoMiR and placebo, the treatment demonstrated significant improvements across multiple measures:
Pain Reduction: Significant reduction in pain at 28 and 90 days was observed on both the numeric rating scale (NRS) and the ASES-E (American Shoulder and Elbow Surgeons Elbow Questionnaire) elbow pain subscale.
Functional Improvement: Significant improvement in upper limb and elbow-specific function was demonstrated at 14, 28 and 90 days using QuickDASH, which assesses the impact of symptoms and disability on daily living, pain and sleep in the upper limb, and patient-rated tennis elbow (搜索) evaluation (PRTEE), which assesses the impact of elbow-specific function on daily living and pain.
Structural Changes: Most notably, the treatment showed significant and sustained improvement in tendon structure over 90 days as measured by greyscale ultrasound, suggesting disease modification rather than merely symptom relief.
Safety Profile
TenoMiR was shown to be safe and well-tolerated both locally and systemically throughout the study period.

No other currently available treatments can predictably improve the microstructure and composition of tendon. This data suggests that this treatment may have efficacy for tendinopathy in other areas, including rotator cuff, patellar tendon and Achilles tendon."
Mechanism of Action
TenoMiR is a chemically synthesized mimic of miR-29a (搜索), developed to target the underlying molecular cause of tendon disease by stimulating natural healing rather than just alleviating symptoms. The therapy is delivered via ultrasound-guided injection into the tendinopathic lesion.
Future Development Plans
Based on these Phase 2 results, Causeway is planning a Phase 3 study of TenoMiR for lateral epicondylitis (搜索) and a Phase 2 study of TenoMiR in patients with rotator cuff tendinopathy (搜索). The company describes tendinopathy (搜索) as representing a huge unmet need for millions of patients worldwide and a significant cost to healthcare systems.
Beyond tendinopathy (搜索), Causeway is exploring the potential of microRNA treatments in additional indications, initially in dermatology and cardiovascular disease, suggesting a broader therapeutic platform based on the microRNA-29a (搜索) mechanism.
