Celltrion Prepares European Launch of Subcutaneous Infliximab Biosimilar Following Positive Regulatory Opinion

The development represents a significant advancement in anti-TNF therapy delivery, offering patients an alternative to traditional intravenous administration.
Strategic Positioning in Treatment Landscape
HoUng Kim, head of Celltrion's medical and marketing division, outlined the company's strategic approach to positioning CT-P13 SC within existing treatment paradigms. For rheumatoid arthritis (search) patients, the subcutaneous formulation is intended as the first choice in second-line treatment, potentially delaying the need to switch to more expensive biologics with different mechanisms of action or JAK inhibitors when first-line anti-TNF-α (search) therapy loses effectiveness.
"With the availability of the CT-P13 SC, patients could now be treated with infliximab SC before switching to other more expensive, different-MOA biologics or JAK-inhibitors," Kim explained.
For inflammatory bowel disease (search) patients, CT-P13 SC could serve as first-line therapy when no existing maintenance treatment problems exist in the patient's treatment pathway. Celltrion is currently seeking IBD indication following results from an ongoing phase 3 study.
Quality of Life and Healthcare System Benefits
The subcutaneous formulation addresses a critical need for improved convenience in infliximab administration. While intravenous anti-TNFs remain necessary for IBD treatment to deliver high dosages during induction, the SC formulation offers potential to transform long-term patient management.
"As IBD patients have to commit to a treatment for years and even decades, patient adherence and compliance, which are closely related to convenience, is a key issue," Kim noted. The subcutaneous formulation aims to release patients from the burden of traveling to treatment centers for IV infusions, offering better quality of life through self-administration capability.
The convenience factor extends beyond patient benefits to healthcare system efficiency. Self-administration reduces demand on healthcare systems by keeping patients out of clinics and providing clinicians additional time for other patients. Celltrion is also developing patient-tailored kits including therapeutic drug monitoring (TDM) and calprotectin testing, allowing patients to monitor disease progression without hospital visits.
Clinical Support and Implementation Strategy
Infliximab's established track record provides a strong foundation for the subcutaneous formulation. With nearly 20 years of real-world experience supporting its safety and efficacy, infliximab has proven effective in treating IBD compared to other biologics, with rapid onset of response. The PANTS study (Personalized Anti-Tumor Necrosis Factor Therapy in Crohn Disease Study) demonstrated infliximab's effectiveness in treating IBD patients compared to other biologics.
Trial investigators at UEG Week 2019 in Spain expressed expectations that CT-P13 SC would provide significant benefits for IBD patients in remission phase when used as maintenance therapy. To build physician experience with CT-P13 SC, Celltrion is planning various clinical studies, including both investigator-initiated trials (IIT) and sponsor-initiated trials (SIT).
Pricing and Market Access Strategy
Unlike traditional biosimilar strategies focused primarily on price competition, CT-P13 SC will not depend solely on pricing competitiveness with other biosimilars. Kim emphasized that the subcutaneous formulation brings additional benefit and value to the healthcare community, justifying its positioning beyond price considerations.
Pricing for CT-P13 SC will vary among countries according to local regulations and pricing and reimbursement systems. Celltrion is committed to working closely with stakeholders including physicians, regulatory agencies, and patient groups to support national authorities in implementing policies relating to infliximab SC, ensuring patient access to the treatment.
Broader Differentiation Strategy
The subcutaneous infliximab development represents part of Celltrion's broader differentiation strategy as biosimilar markets mature. As more biosimilars enter the market and price competition reaches limits, the company is focusing on value-add features for individual biosimilar products in its pipeline.
"According to each biosimilar products' characteristics, we are planning to adapt various device options to improve convenience in administration," Kim stated, indicating that the subcutaneous formulation strategy may extend to other products in Celltrion's portfolio.
CT-P13 was the world's first monoclonal antibody biosimilar, approved by the European Commission in 2013 under the brand name Remsima and by the FDA under the brand name Inflectra. The drug is a biosimilar of MSD's Remicade (infliximab), well-established for treating autoimmune diseases including rheumatoid arthritis (search) and Crohn's disease (search).
