Chemoimmunotherapy Shows Limited Efficacy in Younger Patients with Extensive-Stage Small Cell Lung Cancer

Young patients with extensive-stage small cell lung cancer (search) (ES-SCLC (search)) may not benefit from first-line chemoimmunotherapy (search) to the same extent as older patients, according to findings from a multicenter retrospective study published in BMC Cancer (search). The research challenges the assumption that immunotherapy (search) benefits apply uniformly across age groups in this aggressive malignancy.
The study analyzed 347 ES-SCLC (search) patients, comparing outcomes between a young group (≤45 years, n=59) and a control group (>45 to ≤75 years, n=288). With a median follow-up of 13.7 months, younger patients demonstrated significantly inferior survival outcomes across multiple metrics.
Survival Outcomes Favor Older Patients
In the overall cohort, young patients had shorter progression-free survival (PFS) compared to the control group (median 4.67 vs 5.40 months, P <0.001) and reduced overall survival (OS) (median 13.70 vs 14.37 months, P = 0.028). The survival disadvantage was particularly pronounced among those receiving chemoimmunotherapy (search).
Young patients treated with chemoimmunotherapy (search) showed significantly worse outcomes than their older counterparts, with median PFS of 4.50 versus 5.57 months (P = 0.002) and median OS of 13.20 versus 15.33 months (P = 0.012). The overall response rate was numerically lower in the younger group (68.97% vs 78.70%), though this difference did not reach statistical significance (P = 0.271).
Notably, when comparing chemotherapy (search) alone between age groups, no significant differences emerged in either PFS (4.75 vs 5.37 months, P = 0.082) or OS (13.70 vs 13.73 months, P = 0.407), suggesting the survival disadvantage in young patients is specifically related to immunotherapy (search) response.
No Immunotherapy Benefit in Young Patients
Within the young patient cohort, chemoimmunotherapy (search) failed to demonstrate superiority over chemotherapy (search) alone. Kaplan-Meier analyses revealed no significant differences in PFS (4.50 vs 4.75 months, P = 0.501) or OS (13.20 vs 13.70 months, P = 0.508) between young patients receiving chemoimmunotherapy versus chemotherapy alone.
"Young patients with ES-SCLC (search) exhibited poorer survival outcomes, with shorter PFS and OS compared to the control group," wrote lead study author Lijuan Zhao of the Senior Department of Oncology at The Fifth Medical Center of PLA General Hospital (search) in Beijing, China. "Chemoimmunotherapy (search) may not demonstrate a survival advantage in young patients, as evidenced by similar PFS and OS outcomes compared to chemotherapy (search)."
Increased Toxicity in Younger Patients
Safety profiles differed significantly between age groups. While 86.44% of young patients and 82.97% of older patients experienced treatment-related adverse events, the pattern of toxicities varied considerably.
Young patients had a higher incidence of thrombocytopenia (search) (25.42% vs 14.24%, P = 0.033) and experienced significantly more immune-related adverse events (irAEs) of any grade (30.51% vs 11.46%, P <0.001). Pneumonitis (search) occurred more frequently in the younger group (11.86% vs 2.43%, P = 0.003), as did skin rash (11.86% vs 1.40%, P <0.001).
Two young patients developed grade 3 immune-related complications—one with pneumonitis (search) and another with hepatitis (search)—both requiring immunotherapy (search) discontinuation and steroid treatment. No treatment-related deaths occurred in either group.
Radiotherapy Emerges as Key Prognostic Factor
Cox proportional hazards regression analyses identified radiotherapy as the sole factor significantly associated with improved outcomes in young patients. Univariate analysis showed radiotherapy improved PFS (HR = 0.496, 95% CI: 0.286-0.859, P = 0.012) and OS (HR = 0.207, 95% CI: 0.104-0.411, P <0.001) in the young cohort.
Among all patients receiving chemoimmunotherapy (search), younger age emerged as a negative prognostic factor for both PFS (HR = 0.511, 95% CI: 0.333-0.784, P = 0.002) and OS (HR = 0.556, 95% CI: 0.349-0.885, P = 0.013) in univariate analysis. Liver metastases also negatively impacted outcomes, while radiotherapy provided protective benefits.
Implications for Clinical Practice
The findings suggest fundamental differences in how young and older patients respond to immunotherapy (search) in ES-SCLC (search). The researchers hypothesize that young patients may exhibit greater tumor heterogeneity and invasiveness, contributing to rapid disease progression and resistance to PD-L1 (search) blockade.
"The tumor microenvironment of SCLC, characterized by insufficient immune cell infiltration, particularly a deficiency in CD8+ T-cell (search) infiltration, is highly immunosuppressive and limits the efficacy of chemoimmunotherapy (search)," the authors noted. They suggest young patients with SCLC may be more prone to defective antigen presentation, limiting the effectiveness of immunotherapy (search).
The study's limitations include its retrospective design and relatively small sample size in the young patient group. The researchers acknowledge that the mechanisms underlying poor chemoimmunotherapy (search) efficacy in young ES-SCLC (search) patients remain unclear and warrant further investigation.
"These findings suggest that chemoimmunotherapy (search) may not be as effective in young patients with ES-SCLC (search) and underline the need for tailored treatment strategies in this population," the study authors concluded, emphasizing the importance of age-stratified research in developing optimal treatment approaches for this challenging malignancy.
