Chinese Study Shows Neoadjuvant Therapy Nearly Doubles Disease-Free Survival in Intrahepatic Cholangiocarcinoma

The study, published in the New England Journal of Medicine, represents a significant advancement in treating a disease notorious for its poor prognosis and silent progression.
Multi-Center Trial Design and Patient Population
Led by Zhongshan Hospital (搜索), affiliated with Fudan University, the trial involved 11 hospitals across China and recruited 178 patients with a median age of 59 by February 2024. The study began in 2021 and tested a neoadjuvant protocol—treatment given before the main surgical intervention to shrink tumors.
Patients were divided into two groups: one received three cycles of Gemox (搜索) chemotherapy combined with a targeted therapy drug and an immunotherapy drug that helps the body's immune system recognize and attack cancer cells, while the control group underwent immediate surgery following traditional standard practice.
Dramatic Improvement in Disease-Free Survival
The results showed remarkable efficacy for the combination therapy approach. The median event-free survival—the length of time after treatment that a patient remains free of specific complications or cancer recurrence—reached 18 months for patients receiving the drug cocktail, compared to only 8.7 months for those who went straight to surgery.
"The tumors in the drug-treatment group significantly shrank, with an objective response rate of 55 percent," noted Shi Guoming, one of the lead researchers. This objective response rate refers to the percentage of patients whose tumors partially or completely disappeared during the treatment phase.
Survival Benefits and Long-Term Outcomes
The study also revealed promising trends in overall survival rates. The 24-month survival rate for the drug-protocol group reached 79 percent, substantially higher than the 61 percent observed in the surgery-only group. These findings are particularly significant given that the current five-year survival rate after surgery for ICC ranges from only 25 percent to 40 percent.
Zhou Jian, president of Zhongshan Hospital (搜索), indicated that as the study progresses, it may further demonstrate long-term benefits for patients. The research team presented preliminary results at the annual meeting of the European Society for Medical Oncology in Germany in October 2024, where the findings garnered international attention.
Addressing a Global Health Challenge
Intrahepatic cholangiocarcinoma (搜索) forms in the bile ducts (搜索) inside the liver and is particularly challenging to diagnose early, often being discovered only after reaching an advanced stage. China bears a disproportionate burden of this disease, with over 50,000 new cases annually—accounting for more than half of the global total.
The study's lead researcher Shi expressed hope that the findings will be further validated through international studies so the Chinese protocol can eventually be used as a global treatment approach. Experts consider these findings to offer a new "standard of care" for a disease with historically limited treatment options and poor outcomes.
