Revolutionary Drug Delivery System
The Port Delivery Platform represents an innovative approach to ocular drug delivery, comprising an implantable device designed for long-term, continuous release of medication into the eye. The system includes the implant itself, an insertion device, and ancillary components including initial fill needle, refill needle, and explant tool.
The platform utilizes a customized 100 mg/ml ranibizumab formulation specifically developed for the ocular implant. Ranibizumab is a VEGF inhibitor that binds to and inhibits vascular endothelial growth factor-A (search) (VEGF-A (search)), which has been shown to reduce angiogenesis and vascular leakage in patients with retinal vascular diseases.
Significant Treatment Burden Reduction
The most compelling aspect of this technology lies in its potential to dramatically reduce treatment frequency. While current standard-of-care intravitreal injections require administration every 4 to 16 weeks, the Port Delivery Platform requires refill-exchange only once every 24 weeks, providing sustained efficacy for approximately six months.

The regulatory application is supported by results from two pivotal global Phase III studies: the Archway study in patients with nAMD and the Pagoda study in patients with DME, both conducted by Roche/Genentech in the United States.
Chugai's regulatory strategy involves a two-component approach. The current filing covers the medical device component, while a separate application for the customized ranibizumab drug product is planned for 2026. This second filing will incorporate data from the domestic Phase I/II TEIEN study in Japanese patients with nAMD and DME, alongside the international Phase III results.
Addressing Significant Unmet Medical Need
The target conditions represent substantial global health burdens. Neovascular AMD affects approximately 20 million people worldwide and stands as the leading cause of vision loss in individuals over 60 years of age. The condition causes rapid and severe vision loss when left untreated, developing through uncontrolled growth of abnormal blood vessels under the macula.
Diabetic macular edema (search) affects an estimated 29 million people globally and represents a vision-threatening complication of diabetes. The condition results from persistent hyperglycemia causing blood vessel damage and subsequent fluid leakage into the macula. With increasing diabetes prevalence worldwide, the number of people affected by DME is expected to continue growing.

The Port Delivery Platform addresses a critical challenge in retinal disease management: treatment burden. The current standard requires frequent clinic visits and repeated invasive procedures, creating physical and psychological stress for patients and their families while straining healthcare systems.
