Clinical Evidence Supporting Approval
The regulatory decision was based on results from the TACKLE study, an investigator-initiated Japanese Phase II clinical trial that evaluated Alecensa's efficacy and safety in pediatric and adult patients with rare cancers harboring ALK (search) gene abnormalities, including fusion/rearrangement genes, activating mutations, and gene copy number amplification in advanced or recurrent settings.
In the study's main cohort, the response rate as assessed by the central review committee reached 43.8% (7/16 patients, 95% CI: 19.8%-70.1%) in the full analysis set. More notably, within the ALK (search) fusion gene-positive subpopulation of this cohort, the response rate increased to 70.0% (7/10 patients, 95% CI: 34.8%-93.3%).
The most compelling efficacy data came from the ALK (search) fusion gene-positive subpopulation across all cohorts in the overall full analysis set, which demonstrated a response rate of 76.5% (13/17 patients, 95% CI: 50.1%-93.2%). These results in patients with ALK fusion gene-positive tumors served as the key evidence supporting regulatory approval.
Safety Profile Remains Consistent
The safety analysis revealed an adverse event incidence of 73.1% (19/26 patients), with the most common adverse reactions being decreased lymphocyte count and decreased neutrophil count, each occurring in 23.1% of patients (6/26 patients). Anemia was reported in 19.2% of patients (5/26 patients), while increased blood creatinine occurred in 15.4% of patients (4/26 patients).
Importantly, the safety profile observed in the TACKLE study aligned with Alecensa's previously established safety characteristics, with no new safety signals identified during the investigation.
Companion Diagnostic Approval
Supporting this therapeutic advancement, the MHLW granted approval on March 9, 2026, for the expanded use of FoundationOne CDx Cancer Genomic Profile (search) as a companion diagnostic for Alecensa in ALK (search) fusion-positive solid tumors. This genomic profiling tool enables the identification of patients who could potentially benefit from Alecensa treatment based on their tumor's molecular characteristics.
