Colorectal Cancer Rising in Younger Adults: Surgeons Urge Awareness of Subtle Warning Signs

Colorectal cancer (search), long considered a disease of older adulthood, is now the leading cause of cancer-related death among Americans younger than 50. In response, the American College of Surgeons (search) (ACS) has released free educational materials and a patient checklist designed to help individuals recognize subtle warning signs that are frequently overlooked or attributed to benign conditions.
"A lot of times when younger patients seek care for bowel symptoms, those symptoms are too often attributed to hemorrhoids or constipation and treated conservatively," said Marylise Boutros, MD, FACS, FRCSC, FASCRS, a colorectal surgeon and vice chair of Digestive Diseases Research at the Cleveland Clinic Florida, who helped develop the new ACS materials. "As more young people are diagnosed with colorectal cancer (search), awareness of the subtle signs and symptoms of the disease can save lives."
A Shifting Demographic
According to the American Cancer Society, approximately 1 in 5 colorectal cancer (search) cases are now found in people younger than 54, compared with 11% (about 1 in 10) three decades ago. While the disease still affects about 1 in 25 men and women over a lifetime, the rising incidence among younger adults has prompted urgent calls for heightened clinical suspicion.
The U.S. Preventive Services Task Force now recommends that adults at average risk begin colorectal cancer (search) screening at age 45. However, for symptomatic patients below that threshold, timely diagnosis depends on recognizing warning signs and securing appropriate referrals.
Key Symptoms Not to Ignore
The ACS checklist identifies several symptoms that warrant medical evaluation. Persistent bowel changes—including long-lasting constipation or diarrhea, thin stools, or a clear shift in bowel habits—may signal an underlying issue. "In younger patients, these symptoms can sometimes be due to noncancerous conditions," said Paula Denoya, MD, FACS, FASCRS, a colorectal surgeon at Stony Brook University Hospital. "However, any bowel change that is new or different for you deserves medical attention."
Blood in the stool represents the most critical warning sign. Research presented at the ACS Clinical Congress 2025 found that among patients younger than 50 who underwent colonoscopy because of symptoms, rectal bleeding was the strongest predictor of a colorectal cancer (search) diagnosis, raising the odds by 8.5 times. James T. McCormick, DO, FACS, FASCRS, professor of surgery at Drexel College of Medicine and chair of the Colorectal Cancer Program at Allegheny Health Network, noted: "Bleeding is the most common symptom of colorectal cancer but can also be caused by other diseases. Any amount of recurring blood should be evaluated, and colonoscopy may be indicated to determine the underlying cause."
Additional warning signs include changes in stool appearance—stools that are bright red, dark and tarry, or contain mucus—as well as general health changes such as unexplained weight loss, fatigue, weakness, loss of appetite, nausea, or vomiting.
The Diagnostic Challenge in Younger Patients
The health system was built around the assumption that colorectal cancer (search) is unlikely in the young. Symptoms often overlap with benign conditions: rectal bleeding, abdominal pain, anemia, bowel changes, fatigue, or weight loss may be attributed to hemorrhoids, irritable bowel syndrome, diet, menstrual symptoms, infection, stress, or pregnancy. Studies of early-onset disease have documented red-flag symptoms and common diagnostic delays, as well as primary care barriers that can slow the path to diagnosis.
The stakes of early detection are substantial. NCI SEER data report a five-year relative survival of approximately 91% for localized colorectal cancer (search), compared with about 17% when the disease has spread to distant sites. "Colorectal cancer is highly treatable when found early, and surgical advances continue to make treatment less invasive," Dr. Boutros said.
Beyond Population Averages: Precision Approaches
When early-onset colorectal cancer (search) is diagnosed at an advanced stage, treatment decisions carry implications for time, toxicity, fertility, survivorship, and quality of life. The standard-of-care model, built on population-based clinical trials, shows what works for most patients—but early-onset colorectal cancer exposes the limits of that model.
Genomics has narrowed the therapeutic haystack by identifying mismatch repair deficiency, RAS (search) and BRAF (search) alterations, HER2 (search) amplification, NTRK (search) fusions, and other actionable features. Yet functional precision oncology adds a further dimension: using tissue from biopsy or surgical specimens, living tumor cells can be tested ex vivo against approved drugs and combinations, generating patient-specific drug sensitivity and resistance data.
Clinical feasibility data in relapsed and refractory pediatric cancers, as well as studies implementing patient-derived organoids in advanced colorectal cancer (search), demonstrate why this approach is gaining attention. Artificial intelligence can further strengthen interpretation by helping clinicians synthesize high-dimensional functional and multi-omics data, recognizing patterns of response and resistance faster than manual review alone.
The emerging paradigm is not a replacement for randomized trials or guidelines, but an evidence stack: standard of care, genomics, functional biology, and AI-assisted interpretation. For younger colorectal cancer (search) patients diagnosed late, the goal is to make the therapeutic haystack small enough that the right intervention can be selected with confidence.
