COVID-19 mRNA Vaccines Show Promise in Boosting Cancer Immunotherapy Effectiveness

Cancer (search) patients who received COVID-19 (search) mRNA vaccines (search) alongside immune checkpoint inhibitor therapy demonstrated significantly improved survival outcomes, according to research presented at the European Society for Medical Oncology Congress and published in Nature. The study analyzed medical records from more than 1,000 cancer patients and found that those who received Moderna or Pfizer vaccines within 100 days of starting checkpoint inhibitors were twice as likely to survive three years compared to unvaccinated patients.
Survival Benefits Across Cancer Types
The survival advantages were particularly striking in advanced lung cancer (search) patients, where vaccine recipients had a median survival time of 37 months compared to 21 months for unvaccinated patients. Three-year survival rates reached 56% for vaccinated patients versus 31% for unvaccinated patients. Among metastatic melanoma (search) patients, the median survival was 27 months for unvaccinated patients, while the median had not yet been reached for vaccinated patients because a majority remained alive. Three-year survival rates were 68% for vaccinated patients compared to 44% for unvaccinated patients.
The improvement was especially notable for patients with immunologically "cold" tumors that typically don't respond well to immunotherapy, with these patients experiencing nearly fivefold improvement in three-year survival. Importantly, researchers did not observe the same survival advantage for cancer (search) patients who received non-mRNA vaccines (search), such as flu shots.
Mechanism of Action
Preclinical studies revealed the biological basis for these clinical observations. mRNA vaccines (search) increased production of type I interferon (search), a cytokine that enables innate immune cells to prime the activity of cancer (search)-fighting T cells. Additionally, vaccinated cancer patients showed increased expression of the PD-L1 (search) protein on tumors, which is the target of many immune checkpoint inhibitors (search).
"COVID-19 (search) mRNA vaccines (search) act like alarms, triggering the body's immune system to recognize and kill tumor cells and overcome the cancer (search)'s ability to turn off immune cells," explained University of Florida Health (search) pediatric oncologist Elias Sayour, who led the research. "When combined, vaccines and immune checkpoint inhibitors (search) coordinate to unleash the full power of the immune system to kill cancer cells."
Clinical Implications
The research team discovered that mRNA can train immune systems to kill tumors, even if the mRNA is not related to cancer (search), while developing mRNA vaccines (search) for brain tumor patients in 2016. This finding led to the hypothesis that COVID-19 (search) mRNA vaccines might display tumor-fighting effects.
"This study demonstrates that commercially available mRNA COVID vaccines can train patients' immune systems to eliminate cancer (search)," said Adam Grippin, MD, PhD, of the University of Texas MD Anderson Cancer Center. "When combined with immune checkpoint inhibitors (search), these vaccines produce powerful antitumor immune responses that are associated with massive improvements in survival for patients with cancer."
Future Clinical Development
The research team is preparing to test this treatment strategy in a nationwide clinical trial involving lung cancer (search) patients. Participants receiving immune checkpoint inhibitors (search) will be randomized to either receive a COVID-19 (search) mRNA vaccine during treatment or not. This study aims to determine whether COVID-19 mRNA vaccines (search) should be included as part of the standard of care for patients receiving immune checkpoint inhibitors.
Unlike personalized mRNA cancer (search) vaccines, which can be costly and difficult to manufacture, COVID-19 (search) mRNA vaccines (search) are already widely available at low or no cost globally and could be administered at any time during a patient's treatment. The link between improved survival and COVID-19 mRNA vaccination remained strong even after controlling for factors like disease severity and co-occurring conditions.
Sayour noted that mRNA vaccines (search) not specific to a patient's cancer (search) can "wake up the sleeping giant that is the immune system to fight cancer." The researchers hope this approach will help extend the anti-cancer benefits of immunotherapy to millions of patients who currently lack effective treatment options, particularly those with "cold" tumors that successfully evade immune detection.
