The study, led by researchers from the T1D Exchange Quality Improvement Collaborative (search), analyzed 9,856 children and adolescents aged 18 years and younger diagnosed with type 1 diabetes (search) between 2020 and 2022. Superior Glycemic Outcomes with Early Adoption
Children who started automated insulin delivery (search) within six months of diagnosis achieved a median HbA1c of 7.1% at two years, compared to 7.3% for those starting at 6-12 months, 7.7% for those initiating at 13-24 months, and 8% for non-users (P < .001). This represents a clinically meaningful difference of nearly one percentage point between the earliest adopters and non-users.
"Following the initial HbA1c nadir related to type 1 diabetes (search) honeymoon, glycemic patterns tend to rise and plateau," explained Dr. Elizabeth A. Mann, assistant professor of pediatrics at University of Wisconsin and lead author of the study. "Our findings suggest that using automated insulin delivery (search) systems early after diagnosis may be a strategy to reduce the post-honeymoon HbA1c rise."
The likelihood of achieving optimal glycemic control was substantially higher among early adopters. Youth who initiated automated insulin delivery (search) within six months were 82% more likely to achieve an HbA1c below 7% at two years (OR = 1.82; 95% CI, 1.46-2.27; P < .001) compared to non-users. Even those starting systems later showed benefits, with 40% higher odds for the 6-12 month group (OR = 1.4; 95% CI, 1.15-1.7; P < .001) and 11% higher odds for the 13-24 month group (OR = 1.11; 95% CI, 1-1.23; P < .05).
Reduced Complications and Safety Benefits
The study also demonstrated significant reductions in diabetic ketoacidosis (search) (DKA) rates among automated insulin delivery (search) users. Early adopters experienced 2.3 DKA events per 100 person-years compared to 7.1 events per 100 person-years among non-users (P < .001). The progressive increase in DKA rates with delayed initiation suggests a dose-response relationship between timing and clinical outcomes.
All automated insulin delivery (search) groups showed lower odds of having dangerously high HbA1c levels above 9%, with early adopters being least likely to exceed this threshold (OR = 0.27; 95% CI, 0.18-0.4; P < .001).
Study Population and Methodology
The research encompassed a diverse population with a mean age of 10.1 years and 46% female participants.
