Early Rhythm Control Therapy Remains Effective for Atrial Fibrillation Patients with Obesity and Diabetes

A secondary analysis of the landmark EAST-AFNET 4 trial has revealed that early rhythm control (ERC) therapy maintains its effectiveness and safety profile in atrial fibrillation (search)Search disease (AF) patients with obesity (search)Search disease or diabetes (search)Search disease, challenging traditional treatment approaches for these high-risk populations.

The findings, published in JAMA Cardiology, demonstrate that neither obesity (search)Search disease nor diabetes (search)Search disease significantly impacts the therapeutic benefit of early rhythm control therapy, which reduced cardiovascular complications by approximately 20% compared to usual care over a median follow-up of 5.1 years.

Study Design and Patient Population

The original EAST-AFNET 4 trial enrolled 2,789 patients with AF diagnosed at least one year before enrollment and at least two CHA2DS2-VASc risk factors. Patients were randomly assigned to receive either ERC therapy—consisting of anti-arrhythmic drug therapy (search)Search drug, catheter ablation, or cardioversion—or usual care.

For this secondary analysis, researchers led by Paulus Kirchhof from University Medical Center Hamburg-Eppendorf (search)Search term examined outcomes in 1,086 patients with obesity (search)Search disease (BMI ≥30 kg/m²) compared to 1,690 patients without obesity, and 694 patients with diabetes (search)Search disease versus 2,090 without diabetes.

Comparable Efficacy Across Patient Groups

The analysis revealed that ERC therapy remained equally effective regardless of obesity (search)Search disease status. Among patients with obesity (mean BMI 34.5 kg/m²), 522 received ERC therapy, primarily anti-arrhythmic drug therapy (search)Search drug. The hazard ratios for the composite primary outcome were comparable between groups: 0.84 for patients with obesity and 0.69 for those without obesity.

Similarly, diabetes (search)Search disease status did not significantly alter the treatment effect, with hazard ratios of 0.77 for patients with diabetes and 0.78 for those without diabetes. The primary outcome encompassed death from cardiovascular causes, stroke (search)Search disease, or hospitalization with worsening heart failure (search)Search disease or acute coronary syndrome (search)Search disease.

Safety Profile Remains Consistent

Safety outcomes showed no significant differences between patient groups. For patients with and without obesity (search)Search disease, composite safety outcome rates were 13.4% and 18.5%, respectively. Stroke (search)Search disease rates were 1.1% versus 3.9%, death rates were 7.9% versus 11.2%, and serious adverse events occurred in 5.4% versus 4.5% of patients.

Among patients with diabetes (search)Search disease, the primary safety outcome occurred in 18.2% compared to 16.1% in those without diabetes, with no statistically significant difference observed.

Clinical Implications for Practice

The researchers emphasize that these findings have important implications for clinical care, particularly given the historical reluctance of cardiologists to offer rhythm control therapy to patients with obesity (search)Search disease. Traditional recommendations often suggested achieving weight loss prior to initiating rhythm control therapy.

"The analysis of safety outcomes suggests that established anti-arrhythmic drugs can be safely used in patients with obesity (search)Search disease," the investigators noted. They recommend that "early initiation of rhythm control therapy should be part of the initial management in patients with obesity with recently diagnosed AF."

Patient Characteristics and Comorbidities

Patients with obesity (search)Search disease in the study were younger than those without obesity (mean age 68 vs 72 years) and had higher rates of nonparoxysmal AF patterns (31% vs 24%), arterial hypertension (search)Search disease (94.8% vs 83.3%), and stable heart failure (search)Search disease (32.7% vs 12.0%). However, they had lower rates of stroke (search)Search disease or transient ischemic attack (9.2% vs 13.2%).

Patients with diabetes (search)Search disease were also younger (mean age 69 vs 71 years), more often male (61.0% vs 51.1%), had higher mean BMI (31.3 vs 28.6 kg/m²), and were more likely to have arterial hypertension (search)Search disease (92.8% vs 86.3%) and stage 3 or 4 chronic kidney disease (search)Search disease (16.1% vs 11.4%).

Future Treatment Considerations

The researchers acknowledge that the EAST-AFNET 4 trial was conducted before glucagon-like peptide-1 receptor agonists (search)Search drug became widely available. However, they argue that the "lack of interaction of ERC across the ranges of BMI suggests that ERC will retain its efficacy in patients with obesity (search)Search disease and diabetes (search)Search disease receiving such interventions."

The study authors conclude definitively: "Put simply, neither obesity (search)Search disease nor diabetes (search)Search disease should be a reason to withhold ERC therapy in patients with AF," providing clear guidance for clinicians treating this complex patient population.
