E2086 represents a unique therapeutic approach to narcolepsy (search) treatment by selectively activating orexin (search) 2 receptors. The compound works by enhancing orexin receptor activity and acting on the underlying pathophysiology of narcolepsy, particularly in type 1 narcolepsy where autoimmune destruction of orexin-producing neurons in the hypothalamus leads to orexin deficiency.

More importantly, Phase Ib clinical trial data presented at the World Sleep 2025 congress demonstrated that E2086 achieved a statistically significant reduction in excessive daytime sleepiness compared to both placebo and the existing standard treatment modafinil in patients with narcolepsy (search) type 1.
The clinical results suggest E2086 has the potential to improve daytime wakefulness in narcolepsy (search) patients, addressing one of the most debilitating symptoms of the condition.
