EMA Recommends Amgen's Uplizna for Rare Autoimmune Disease IgG4-RD

The European Medicines Agency (search)'s (EMA) human medicines committee (CHMP) has recommended extending the therapeutic indication of Amgen Inc.'s Uplizna (inebilizumab) to include the treatment of adult patients with active immunoglobulin G4-related disease (search) (IgG4-RD (search)), marking a potential breakthrough for patients with this rare autoimmune condition.
Addressing an Unmet Medical Need
IgG4-RD (search) is a rare, chronic, autoimmune condition caused by the body's own defense system attacking normal tissue, which can cause fibrosis (tissue scarring) and inflammation in one or multiple organs. Currently, there are no authorized medicines in the EU specifically for adults with IgG4-RD, and patients are commonly treated with glucocorticoids (search) and other immunomodulatory medicines.
Uplizna contains inebilizumab, a monoclonal antibody that attaches to immune cells called B cells (search) and destroys them. The drug is already approved to treat patients with neuromyelitis optica spectrum disorders (search) (NMOSD (search)).
Phase 3 Trial Results
The CHMP based its recommendation on the assessment of efficacy and safety data from a phase 3 trial involving 135 adult patients with active IgG4-RD (search). Patients received either intravenous Uplizna or matching placebo on day 1, day 15, and week 26.
The trial demonstrated significant clinical benefits for patients receiving Uplizna. The median time to the first treated IgG4-RD (search) flare was significantly longer in patients receiving Uplizna compared to placebo. Of the 68 patients receiving Uplizna, only seven were treated for IgG4-RD flares, compared to 40 of the 67 patients receiving placebo.
Superior Remission Rates
The most striking finding was the achievement of corticosteroid-free, flare-free complete remission. At week 52, 58.8% of patients receiving Uplizna achieved this endpoint, defined as a period without disease symptoms after treatment, compared to 22.4% of patients receiving placebo.
This represents a more than two-fold improvement in complete remission rates, potentially offering patients with IgG4-RD (search) a chance to avoid long-term corticosteroid use and its associated side effects while maintaining disease control.
