Ensifentrine Demonstrates Sustained Improvement in COPD Symptoms and Quality of Life in Stable Patients

The findings, presented at the CHEST Annual Meeting, suggest the FDA-approved maintenance therapy could address persistent symptom burden in patients who continue to experience daily symptoms despite standard bronchodilator therapy.
Sustained Dyspnea Relief Across Study Period
The analysis examined 1,384 adults aged 40 to 80 years from the ENHANCE-1 and ENHANCE-2 trials who did not experience moderate or severe COPD (search) exacerbations during the 24-week study period. Of these patients, 891 received 3 mg twice-daily nebulized ensifentrine while 493 received placebo, with both groups showing comparable baseline characteristics including age, gender, dyspnea (search) index scores, and respiratory symptom measures.
Patients receiving ensifentrine demonstrated significant improvements in Transition Dyspnea (search) Index (TDI) scores compared to placebo at all measured time points. At 6 weeks, the least-square mean TDI score was 1.4 for ensifentrine versus 0.6 for placebo, improving to 1.6 versus 0.8 at 12 weeks, and reaching 2.1 versus 1.0 at 24 weeks (P < 0.05 for all comparisons).
The proportion of patients achieving the minimal clinically important difference (MCID) of at least 1 unit was consistently higher in the ensifentrine group: 56% versus 39% at week 6 (OR = 1.7; 95% CI, 1.3-2.2), 61% versus 42% at week 12 (OR = 1.7; 95% CI, 1.4-2.2), and 66% versus 45% at week 24 (OR = 1.9; 95% CI, 1.5-2.4).
Comprehensive Symptom Improvement
Respiratory symptoms, measured by the Evaluating-Respiratory Symptoms (E-RS) total score, showed significantly greater reductions with ensifentrine treatment. The least-square mean changes from baseline were -2.0 versus -0.8 at 6 weeks, -2.4 versus -1.2 at 12 weeks, and -2.4 versus -1.5 at 24 weeks compared to placebo.
More patients in the ensifentrine group achieved the E-RS MCID of ≥2-point reduction at week 6 (43% vs. 33%; OR = 1.5; 95% CI, 1.2-2.0) and week 12 (48% vs. 36%; OR = 1.7; 95% CI, 1.3-2.2). At week 24, the proportion remained numerically higher for ensifentrine (46% vs. 41%).
Quality of Life Benefits
Health-related quality of life, assessed using the St. George's Respiratory Questionnaire (SGRQ), improved significantly with ensifentrine at early time points. The least-squares mean changes were -5.5 versus -3.2 at week 6 and -5.8 versus -3.7 at week 12 compared to placebo.
Significantly more patients receiving ensifentrine met the SGRQ MCID of ≥4-point reduction at week 6 (48% vs. 41%; OR = 1.4; 95% CI, 1.1-1.7) and week 12 (50% vs. 41%; OR = 1.4; 95% CI, 1.1-1.8). At week 24, the advantage remained numerically favorable (53% vs. 50%).
Clinical Implications for COPD Management
"As clinicians, we see many patients with COPD (search) who continue to have symptoms despite reducing exacerbation with standard of care therapies, [and] our results suggest that an added benefit can be achieved with ensifentrine nebulization," said Nicola A. Hanania, MD, MS, director of the Airways Clinical Research Center at Baylor College of Medicine and lead investigator.
The study confirms that COPD (search) patients without exacerbations still experience significant daily symptom burden and impaired quality of life requiring therapeutic intervention. Ensifentrine's dual mechanism of action, blocking both PDE4 (search) and PDE3 (search), produced early and sustained improvements across multiple patient-reported outcome measures.
Treatment-emergent adverse events showed no difference between ensifentrine and placebo groups in the analyzed patient population, supporting the therapy's safety profile in stable COPD (search) patients.
Future Research Directions
The investigators suggest future COPD (search) studies should focus on Group B patients who remain symptomatic despite bronchodilator therapy, even when not at high exacerbation risk. This patient population represents an important unmet medical need in COPD management, where symptom control rather than exacerbation prevention becomes the primary therapeutic goal.
The consistent pattern of improvement across dyspnea (search), respiratory symptoms, and quality of life measures provides convergent evidence that ensifentrine offers meaningful clinical benefits for everyday function in stable COPD (search) patients, potentially complementing existing maintenance regimens for those who continue to experience persistent symptoms.
