The collaborative effort brought together physicians from across the United States to develop practical guidelines for CLL (search) treatment. Dr. Deborah Stephens, Director of the CLL and Richter's Program at the University of North Carolina School of Medicine (search), shared insights about the workshop's key discussions and areas of ongoing debate.
Emerging Biomarkers in Clinical Practice
While the workshop achieved consensus on many topics, certain areas remained under active discussion. The clinical utility of minimal residual disease (MRD) testing emerged as a key point of deliberation, with experts debating its role in guiding therapy decisions. Similarly, the use of BTK inhibitor (search) resistance mutations as treatment determinants was deemed not yet ready for mainstream clinical practice, though multiple ongoing trials are investigating their potential application.
"Since there is not a consensus, we decided that it's not quite ready for mainstream [practice], but there is a lot of clinical trials ongoing that hope to help address how to best use this in clinical practice," explained Dr. Stephens.
Emphasis on Shared Decision-Making
Dr. Jacob D. Soumerai, Clinical Investigator in Lymphoma at Massachusetts General Hospital, emphasized the critical importance of patient involvement in treatment decisions. Recent data revealed a concerning disparity: while most CLL (search) patients express a desire to participate in treatment discussions, less than half report having such opportunities.
"One thing that really resonated for me was the fact that we have so many factors that drive the decision-making. But at the end of the day, probably for a majority of patients, there are multiple reasonable treatment approaches," noted Dr. Soumerai.
Bridging the Communication Gap
The workshop's recommendations aim to address this communication gap by providing comprehensive resources for both clinicians and patients. The goal is to ensure that all stakeholders have access to the necessary information for making informed treatment decisions.
"This effort is attempting to both provide the clinicians with all of the data that we use in making these treatment decisions but also trying to empower patients to come to these discussions with as much information as possible," Dr. Soumerai added.
The initiative represents a significant step forward in standardizing CLL (search) treatment approaches while acknowledging the importance of individualized care and patient preferences in treatment planning.
