FDA Approves Larger Vial Size of Nelarabine Injection for T-Cell Leukemia and Lymphoma Treatment

The FDA has granted approval to Shorla Oncology (search) for a larger vial size of nelarabine injection (SH-111), expanding treatment options for adult and pediatric patients with T-cell acute lymphoblastic leukemia (search) (T-ALL) and T-cell lymphoblastic lymphoma (search) (T-LBL). The new 375mg/75mL vial represents the company's second FDA approval for this nucleoside metabolic inhibitor, following the initial approval of a 250mg/50mL vial in 2023.
Addressing Clinical Dosing Challenges
The larger vial size specifically targets the dosing disparities between adult and pediatric populations with T-cell malignancies. According to Shorla Oncology (search), pediatric T-ALL patients have a median age of diagnosis at nine years with an average body surface area (BSA) of 1.07 m², requiring approximately 696 mg doses. In contrast, adult patients typically have an average BSA of 1.7 m² and require higher nelarabine doses of approximately 2,550 mg.

Clinical Efficacy and Safety Profile
Nelarabine injection is indicated for treatment of adult and pediatric patients aged 1 year and older with relapsed or refractory disease following treatment with at least 2 chemotherapy regimens. The drug received accelerated approval from the FDA in 2005 based on data from two single-arm clinical trials.
Clinical trial results demonstrated complete response (CR) rates of 18% in adults and 13% in pediatric patients. When including complete response with incomplete hematologic or bone marrow recovery (CRi), response rates reached 21% and 23% for adult and pediatric populations, respectively.
Safety Considerations
Nelarabine carries a boxed warning for neurologic toxicity, including peripheral neuropathy (search), which was identified as dose-limiting in both age groups. The prescribing information warns of severe neurologic adverse reactions including altered mental states with severe somnolence, central nervous system effects including convulsions, and peripheral neuropathy ranging from numbness and paresthesias to motor weakness and paralysis.
Additional severe toxicities include hematologic, hepatic, and metabolic laboratory abnormalities in pediatric patients, and gastrointestinal and pulmonary toxicities in adult patients. The most common adverse reactions (≥20%) in adults include anemia, thrombocytopenia, neutropenia, nausea, diarrhea, vomiting, constipation, fatigue, pyrexia, cough, and dyspnea.
