FDA Confirms Single Pivotal Study Standard for Drug Approval, Accelerating Cinclus Pharma's GERD Treatment Path

This shift away from the historically established requirement for two pivotal studies per indication represents a major change in FDA policy, as announced in the New England Journal of Medicine.
The regulatory update has immediate implications for Cinclus Pharma (search) Holding AB, a late-stage clinical pharmaceutical company developing next-generation treatments for acid-related gastrointestinal diseases (search). The company's current Phase III program for linaprazan glurate in erosive gastroesophageal reflux disease (search) (GERD) now meets the requirements for regulatory submission in both the US and Europe.

Regulatory Pathway Streamlined
Cinclus Pharma (search) initiated its Phase III program for linaprazan glurate in autumn 2025 with an initial healing study in Europe, where two pivotal studies are still required for healing of erosive GERD (search). Topline results from this study are expected in H2 2026 and will support the US regulatory submission. The program will be completed with a second healing study, planned in both the US and Europe, where patients will also be evaluated for maintenance treatment as required by both the FDA and EMA.
The FDA has also provided positive assessment of the company's nonclinical development plan following a scientific advice meeting. The Agency expressed support for Cinclus Pharma (search)'s nonclinical program, including the toxicology studies required ahead of a future New Drug Application (NDA), and concluded that no additional toxicological studies of linaprazan glurate are needed.
"The FDA's positive assessment is a clear confirmation of the quality of our research and nonclinical program. The fact that the Agency does not see a need for any further toxicology studies brings both time and resource efficiencies and represents an important step toward our goal of offering patients a new and much-needed treatment," Ahlberg noted.
Next-Generation GERD Treatment
Linaprazan glurate is a next-generation potassium-competitive acid blocker (search) (PCAB (search)) that offers superior acid control compared with existing treatment options. The drug candidate, originally developed by AstraZeneca, has the potential to heal erosions in the esophageal mucosa and relieve symptoms of GERD more effectively than current treatments like proton pump inhibitors (search) (PPI).
The safety and efficacy of linaprazan and linaprazan glurate have been documented in over 30 phase I and two phase II studies involving more than 3,000 participants. The drug is specifically designed to address the medical needs of patients suffering from the more severe forms of erosive GERD (search).
Addressing Significant Medical Need
GERD affects approximately 133 million adults in the US and EU, with a significant need for new drugs to treat the most severe cases affecting around 10 million patients. The regulatory precedent for single-study approval already exists, as the PCAB (search) that has received marketing authorization in the US did so on the basis of one pivotal Phase III study conducted in the US.
The FDA's new standard for granting marketing authorization based on a single adequate and well-controlled study, in combination with confirmatory evidence, had previously been accepted for certain medicinal products on a case-by-case basis. This formalization of the approach represents a broader shift in regulatory thinking that could accelerate drug development timelines across the industry.
