FDA Lifts Clinical Hold on Blue Lake's Pediatric RSV Vaccine Trial, Allowing Enrollment of RSV-Negative Toddlers

Blue Lake Biotechnology (search) has secured written agreement from the U.S. Food and Drug Administration (FDA) to resume dosing of BLB201, its investigational RSV (search) vaccine, in children as young as 18 months regardless of their RSV exposure status. The decision clears a significant regulatory hurdle for what the company describes as the most advanced pediatric RSV vaccine for young children currently in clinical development.
FDA Lifts Partial Clinical Hold
The FDA had previously placed clinical holds on pediatric RSV (search) vaccine trials in December 2024, excluding enrollment of children who tested negative for RSV exposure or were under two years old. These holds resulted from a safety signal found in another company's clinical trial of an mRNA-based RSV vaccine, where suspected cases of vaccine-associated enhanced respiratory disease (search) (VAERD) were reported.

Promising Efficacy Data
Prior to the clinical hold, Blue Lake had enrolled 63 participants in the Phase 1/2a study of BLB201, ranging from 8 to 59 months old, with over one-third being RSV (search)-negative. A preliminary analysis of partially unblinded data in December 2024 demonstrated that children who received BLB201 were 80% less likely to contract symptomatic RSV infections compared to those who received placebo (p<0.05).
The vaccine has shown excellent tolerability and safety across all participants enrolled in clinical studies to date. During the partial clinical hold period, Blue Lake was permitted to continue enrolling RSV (search)-positive children over two years old.
Expanding Trial Population
The lifting of the partial clinical hold follows a recent Type C meeting with FDA and enables enrollment of children as young as 18 months regardless of RSV (search) serostatus. This development allows for continued development of BLB201 for use in toddlers and makes possible future development in even younger children, including infants.

Addressing Unmet Medical Need
Respiratory syncytial virus (RSV (search)) is a highly contagious respiratory virus and one of the leading causes of acute respiratory disease (search), infecting more than 64 million people worldwide per year. According to the Centers for Disease Control, up to 80,000 children under five years old and up to 180,000 older adults are hospitalized annually due to RSV infection in the US, with up to 10,000 deaths each year.
While approved antibody drugs and a maternal RSV (search) vaccine can provide passive immunity against RSV to infants, there is no approved vaccine for generating prophylactic immunity against RSV in infants and children.
About BLB201
BLB201 is an intranasal RSV (search) vaccine candidate that encodes the RSV F protein (search) and uses a proprietary PIV5 (search) vector, which is not known to cause human disease. The vaccine has received Fast Track designation from the FDA for the prevention of RSV-associated acute respiratory disease (search) in adults over 60 years and pediatric populations.
Phase 1 clinical trial results demonstrate BLB201's safety and ability to induce antibody and cell-mediated immune responses.
