First Clinical Study Demonstrates Feasibility of Fluorescence Imaging in Orthopaedic Surgery

A landmark feasibility study published in BMJ Open has demonstrated for the first time that real-time fluorescence imaging can be used during orthopaedic surgery to detect residual bacterial burden following standard surgical debridement. The research, conducted independently by surgeons at The Royal Orthopaedic Hospital NHS Foundation Trust (ROH) in Birmingham, England, evaluated the MolecuLightDX (search) imaging device in 10 patients undergoing revision knee arthroplasty for periprosthetic joint infection (search).
Periprosthetic joint infection (search) remains one of the most serious complications following joint replacement surgery. Radical surgical debridement — the complete removal of infected tissue — is critical to reducing the risk of persistent or recurrent infection, yet identifying all areas of bacterial contamination intraoperatively continues to pose a significant challenge for orthopaedic surgeons.
Study Design and Key Findings
In this prospective pilot study, surgeons at the ROH performed standard surgical debridement and then used MolecuLight (search) fluorescence imaging to assess the operative field for any remaining bacterial burden. When areas exhibiting bacterial fluorescence were identified that had not been detected during the initial debridement, these regions were subsequently biopsied. Microbiological analysis confirmed the presence of the same bacterial species responsible for the periprosthetic joint infection (search), validating the fluorescence signal.
"This feasibility study is suggestive that the MolecuLight (search) device may assist surgeons in improving their radical debridement at the time of revision surgery of the knee for prosthetic joint infection, which is considered the key step to minimising the morbidity of recurrent infection," said Professor Jonathon Stevenson, senior author and Consultant Orthopaedic Surgeon at the Royal Orthopaedic Hospital, Birmingham.

Mr James Archer, Speciality Registrar in Trauma and Orthopaedics at the ROH and the study's first author, echoed the cautious optimism: "This feasibility study provides the first work to demonstrate that MolecuLight (search) may be able to aid surgeons intra-operatively during revision surgery for periprosthetic joint infection (search) of the knee. We look forward to performing larger studies to examine this in more detail."
