Dr. Philip Kuriakose, chief of hematology at Henry Ford Hospital and medical director of the Hemophilia and Thrombosis Treatment Center, reported positive early results. "Right now, things are going exactly as we expect. Things are going well," Kuriakose said. "What we hear, he is very happy. In his mind, he's crossed a hurdle. It's quite a big step in many ways."
Revolutionary Treatment Approach
Roctavian represents the first FDA-approved gene therapy for adults with severe hemophilia A (search) who do not have antibodies to the AAV5 (search) virus. The treatment uses a modified virus as a vector to deliver a working copy of the factor VIII (search) gene directly to liver cells, enabling the body to produce clotting factor independently.
"The gene goes into the patient's liver and it becomes a part of the patient's liver and starts living there and making more protein," Kuriakose explained. "This protein that is made, the factor VIII (search) in this instance, is made at a steady level. So there are none of these ups and downs. No peaks and valleys."
The therapy aims to transform patients with severe hemophilia requiring frequent infusions into those with mild hemophilia, only needing treatment during surgery or after injuries. Kuriakose anticipates that after six months, as levels stabilize and side effects remain minimal, patients "can get back into much of life, like most of us do, without thinking twice."

Hemophilia A (search) affects approximately 1 in every 5,000 male births, with about 33,000 males in the United States living with hemophilia according to CDC data from 2012-2018. Current treatment typically involves regular at-home injections or infusions of replacement clotting factor, potentially several times weekly. For patients with severe hemophilia, clotting factor therapies can cost approximately $300,000 annually, with total medical expenses reaching twice that amount.
Data released in June by BioMarin Pharmaceutical, Roctavian's manufacturer, showed promising long-term results. Eighty-one percent of participants in hemophilia gene therapy studies remained off prophylaxis five years after treatment. Recent data indicates patients still maintain adequate factor levels with minimal or no ongoing side effects five to seven years post-treatment.
Economic Considerations and Access
While gene therapy represents a significant upfront investment, with each vial of Roctavian costing $94,745, the total treatment cost for an average 175-pound patient requiring 30 vials exceeds $2.8 million. However, Kuriakose suggests that over a lifetime, gene therapy might prove more cost-effective than continuous traditional treatments.
Rox Gatia, vice president of pharmacy shared services at Henry Ford Health (search), noted that insurance coverage remains restrictive. "Most insurance companies have very strict guidelines on gene therapies, especially for cellular and gene editing, and who can access them, so not everyone can get them," Gatia explained. The recent Michigan patient's treatment was covered by insurance.
Expanding Gene Therapy Program
Henry Ford Health (search) has developed comprehensive infrastructure to support gene therapy delivery over the past four years, including dedicated cell and gene therapy pharmacy specialists. The program extends beyond hematology to oncology, rare genetic diseases, and other emerging therapies.
The health system plans to treat sickle cell patients with gene therapy within the next 12-18 months and continues evaluating patients for various gene therapies. Gatia emphasized the broader implications: "We're talking about hemophilia today, but there's potential to cure chronic disease, I mean, let alone a rare disease or a genetic disease or a cancer."
Treatment Process and Monitoring
The recent Michigan patient's infusion took over two hours to complete, though duration varies among patients. Post-treatment monitoring includes regular blood draws to ensure proper response and monitor for side effects, particularly liver-related complications.
Kuriakose emphasized that while gene therapy doesn't cure hemophilia, it provides effective long-term control. "You're not curing it with gene therapy, you're controlling it in that patient, and allow for a lifetime of, hopefully, healthy living without toxicity."
