Capsida Biotherapeutics (search) confirmed in January 2026 that cerebral edema (search) was the cause of death for the first patient in its suspended SYNRGY clinical trial testing CAP-002 for STXBP1 (search), a rare genetic disorder.
Autopsy Findings Reveal Limited Answers
The autopsy confirmed cerebral edema (search) as the cause of death but failed to establish what triggered the brain swelling. "Cerebral edema was an unexpected outcome that was not observed in this program's safety studies that supported the trial," Capsida stated. The company emphasized that the trial was conducted according to protocol with no evidence of clinical trial error, and that the gene therapy product was appropriately manufactured, handled, and dosed.
The SYNRGY trial was designed to test CAP-002, an AAV gene therapy for STXBP1 (search), a rare genetic disorder characterized by epilepsy (search) and encephalopathy (search). The trial was halted in September 2025 after the patient died two and a half days following intravenous treatment.
Expert Skepticism and Calls for Transparency

Retired gene therapy physician John Shoffner commented that the update "isn't informative" and called for transparency regarding clinical and laboratory findings as well as AAV gene therapy impurities. "This is one of many examples where detailed results that can be assessed by many experts would benefit the patients and the field," Shoffner stated.
Imperial College London professor of neurology Pankaj Sharma noted the complexity of determining causation. "The most common cause of death [in STXBP1 (search) patients] is sudden unexpected death in epilepsy (search), followed by respiratory issues," Sharma explained. "It is possible that the gene therapy contributed to death, but it is also possible that the patient's death is unrelated."
Unique Delivery Method Raises Questions
CAP-002's intravenous delivery method, designed to mitigate neurotoxicity risks compared to direct central nervous system injection, may have introduced other complications. "Usually, IV delivery would be anticipated to require a higher viral titer load when compared to direct CNS injection," Sharma said. "High systemic doses could trigger an immune response that in theory contributes to cerebral inflammation leading to cerebral edema (search)."
However, Sharma cautioned against premature conclusions, noting that "in rare cases, uncontrollable seizures can lead to edema."

The death has sent ripples of concern through the gene therapy research community, particularly those developing treatments for brain disorders.
