GLP-1 Drugs Show 16% Reduction in Epilepsy Risk Among Diabetes Patients in Large-Scale Study

A large-scale observational study has revealed a potential unexpected benefit of GLP-1 drugs, showing a 16% reduction in epilepsy (search) risk among people with type 2 diabetes (search) compared to those taking alternative diabetes medications. The findings, published December 10, 2025, in Neurology, the medical journal of the American Academy of Neurology, suggest these widely-used diabetes and weight loss medications may offer neurological protection beyond their established metabolic benefits.
Study Design and Population
Researchers analyzed data from a U.S. health database encompassing 452,766 adults with type 2 diabetes (search) who had no prior history of epilepsy (search) or seizures. The participants, with an average age of 61, were divided equally between those prescribed GLP-1 drugs and those receiving dipeptidyl peptidase-4 inhibitors (DPP-4 inhibitors (search) or gliptins) as comparator medications. The GLP-1 drugs included in the analysis were dulaglutide, liraglutide, and semaglutide.
Over a minimum five-year follow-up period, researchers tracked epilepsy (search) development in both groups. The study found that 1,670 people taking GLP-1 drugs developed epilepsy (2.35%), compared to 1,886 people taking DPP-4 inhibitors (search) (2.41%).
Adjusted Risk Analysis
After accounting for confounding factors including age, high blood pressure, and cardiovascular disease, the research team determined that patients taking GLP-1 drugs were 16% less likely to develop epilepsy (search) than those using DPP-4 inhibitors (search). When examining individual medications within the GLP-1 class, semaglutide demonstrated the strongest association with reduced epilepsy risk.
"Additional randomized, controlled trials that follow people over time are needed to confirm these findings, but these results are promising, since people with diabetes are at increased risk for developing epilepsy (search) later in life," said study author Edy Kornelius, MD, PhD, of Chung Shan Medical University in Taichung, Taiwan.
Clinical Significance and Implications
The potential neurological benefits of GLP-1 drugs carry particular significance given the substantial burden of epilepsy (search) among diabetes patients. Dr. Kornelius emphasized the clinical importance of these findings, noting that "epilepsy can have many physical, psychological and social consequences, and many people do not respond to the current medications, so finding ways to reduce this risk is critical."
The researcher cautioned that the findings support theories about GLP-1 drugs' neurological benefits beyond blood sugar control but stressed that "these findings do not imply that DPP-4 inhibitors (search) are harmful in any way or that GLP-1 drugs are definitely beneficial for brain health."
Study Limitations and Future Research
The observational, retrospective design of the study presents inherent limitations that prevent establishing causation. The research team acknowledged several factors not captured in their analysis, including family history of epilepsy (search), genetic susceptibility, and alcohol use patterns. Additionally, variables such as medication cost, insurance coverage restrictions, and diabetes severity could have influenced drug prescribing patterns, potentially creating unmeasured differences between the study groups.
Notably, tirzepatide, a dual GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) receptor agonist, was excluded from the analysis as it became available after the study period commenced. Consequently, these findings may not extend to tirzepatide treatment.
The study authors emphasize that randomized, controlled clinical trials will be necessary to confirm whether GLP-1 drugs directly reduce epilepsy (search) risk or if the observed association reflects other factors. Such trials would provide the definitive evidence needed to establish causation and inform clinical practice guidelines for diabetes patients at risk of developing epilepsy.
