GLP-1 Receptor Agonists Reduce Asthma Exacerbations by 49% in Adolescents with Obesity

A large retrospective cohort study has revealed that glucagon-like peptide-1 receptor agonists (GLP-1RAs) are associated with a significantly lower risk of acute asthma exacerbations in adolescents with overweight or obesity. The findings, published in JAMA Network Open, represent the first evidence of this association in the adolescent population and suggest potential dual benefits for both weight management and asthma control.

Study Design and Population

Researchers conducted a retrospective analysis using data from the TriNetX (search)Search term global federated health research network between 2020 and 2025. The study included adolescents aged 12 to 18 years with diagnoses of asthma and overweight or obesity, with or without diabetes.

After 1:1 propensity score matching based on baseline demographic characteristics, BMI categories, asthma severity proxies, and prior medication use, 1,070 adolescents were included in the final analysis. The study population had a mean age of 15.8 years and was 56.8% female, with 535 adolescents receiving new GLP-1RA prescriptions and 535 receiving nonpharmacological weight management interventions as controls.

Significant Reduction in Asthma Exacerbations

During 12 months of follow-up, adolescents treated with GLP-1RAs experienced significantly fewer acute asthma exacerbations compared to controls, with only 5.4% versus 10.7% experiencing exacerbations. This corresponded to a relative risk reduction of approximately 49% (RR = 0.51; 95% CI, 0.33-0.78; P = .002).

The benefits extended beyond exacerbation prevention. GLP-1RA use was associated with significantly lower rates across multiple asthma-related outcomes:

• Inhaled short-acting beta-2 agonist use: 32.3% versus 44.7% (RR = 0.72; 95% CI, 0.62-0.84; P < .001)
• Systemic corticosteroid prescriptions: 20.7% versus 31.4% (RR = 0.66; 95% CI, 0.54-0.81; P < .001)
• Asthma-related emergency department visits: 1.5% versus 3.6% (RR = 0.42; 95% CI, 0.19-0.95; P = .04)

However, among adolescents who experienced at least one exacerbation, the number of subsequent events over the follow-up period did not differ significantly between groups, with means of 1.83 versus 2.02 events.

Clinical Context and Implications

"For everyday clinicians, the findings highlight a potential consideration for a subset of adolescents with obesity whose asthma is particularly difficult to control and for whom weight reduction remains challenging," said Lin-Shien Fu, MD, chief of pediatric nephrology and immunology at Taichung Veterans General Hospital and lead author of the study.

The research addresses a significant clinical need, as obesity is a well-established risk factor for poorer asthma control, increased symptom burden, and more frequent exacerbations in adolescents. With GLP-1RAs increasingly prescribed for weight management in this age group following FDA approvals for adolescents aged 12 years and older in 2020 and late 2022, understanding their potential effects on asthma outcomes has become increasingly important.

Potential Mechanisms

The findings suggest a potential dual benefit of GLP-1RAs in adolescents with obesity-related asthma, addressing both weight management and asthma exacerbation risk. However, whether the observed association reflects weight loss, weight-independent anti-inflammatory effects, or metabolic improvements remains unclear and requires further investigation.

"The findings were not entirely unexpected. Prior studies in adults have suggested that GLP-1 receptor agonists may be associated with improved asthma control and a reduced risk of asthma-related outcomes," Fu explained. "However, because childhood asthma is predominantly driven by type 2 immune pathways and its pathophysiology differs from that of adult asthma, it remained important to examine whether similar associations would be observed in pediatric populations using real-world data."

Study Limitations and Future Directions

As an observational study, the findings cannot establish causality and may be affected by residual confounding. The reliance on a predominantly US-based database may also limit generalizability. Additionally, researchers noted they were unable to precisely capture individual weight-loss outcomes in the adolescent population with obesity.

"Future studies incorporating more detailed longitudinal weight data will be needed, and cohort studies or randomized controlled trials may help further clarify the underlying mechanisms and better define the clinical implications of GLP-1 receptor agonist use in this population," Fu said.

The researchers emphasized that prospective randomized clinical trials are needed to confirm efficacy, clarify mechanisms, and assess safety before GLP-1RAs can be considered as adjunct therapy for asthma in this population. Nevertheless, the study provides important early real-world evidence that may inform clinical decision-making for this difficult-to-treat population.
