According to the Endocrine Society, an estimated 5-6 million women of reproductive age in the U.S. are affected by PCOS (search), which is frequently underdiagnosed. The hormonal condition, caused by high levels of testosterone and androgens (search), leads to symptoms including irregular periods, excess hair growth, acne, and insulin resistance (search). Currently, there is no standard treatment for PCOS, with women typically relying on birth control, lifestyle changes, and metformin to manage symptoms.
Clinical Evidence Emerges
A 2023 study published in the Journal of Clinical Medicine provided compelling evidence for semaglutide's effectiveness in PCOS (search) patients. The research examined 27 women with PCOS and obesity (search) who had not achieved weight reduction through lifestyle modifications alone. Participants received 0.5 mg of subcutaneous semaglutide weekly for three months, with responsive patients continuing treatment for an additional three months.
The results were significant: nearly 80% of patients achieved at least a 5% decrease in body weight after three months, with a mean weight loss of 7.6 kg and BMI reduction of 3.1. For patients who continued treatment for six months, the mean body weight loss reached 11.5 kg, with BMI decreasing from 34.4 to 29.4. Crucially, 80% of responsive patients experienced normalized menstrual cycles while on semaglutide treatment.
"Independently of results on body weight, and in patients who did not obtain a 5% decrease in their body weight, insulin basal values decreased, and the Homeostatic Model Assessment for Insulin Resistance (search) improved," the study authors noted. Fasting blood glucose normalized in approximately 80% of treated women who had elevated glucose levels at baseline.
Patient Experiences Drive Interest
Individual patient experiences are reinforcing the clinical findings. Grace Hamilton, a 27-year-old woman who participated in a GLP-1 trial for PCOS (search) patients, reported dramatic improvements after receiving her first dose of semaglutide in October 2024. Hamilton had previously struggled with hair loss, heavy periods, inconsistent menstrual cycles, and mental health issues despite trying birth control and lifestyle changes.
"I'm so glad that I stuck with it because it was just clear as day that it was the missing link for me," Hamilton said, describing almost immediate results including resumed periods, hair regrowth, and improvement in depression and anxiety.
Broader Survey Data Supports Findings
A recent survey of 1,700 people by the birth control app Natural Cycles revealed that 64% of women with PCOS (search) who took GLP-1 medications reported more predictable periods, while 20% reported shorter bleeding days. These real-world observations align with the controlled clinical trial results.
Mechanism of Action in PCOS

Dr. Melanie Cree, a pediatric endocrinologist at Children's Hospital Colorado who has been studying GLP-1 effects in adolescents with PCOS (search) for over a decade, emphasizes the significant unmet need in PCOS treatment. "Primary therapies used for PCOS symptoms haven't changed in nearly 50 years," Cree noted.
However, current research has limitations.
