Telehealth platforms emerged as a critical access channel, offering convenient prescriptions and more affordable compounded versions during national shortages. Yet by 2025–2026, the sector faced a sharp pivot as FDA actions resolved shortages, curtailed large-scale compounding, and issued warning letters targeting misleading marketing claims.
The Compounding Boom and Lower-Cost Models
During the height of shortages, compounded GLP-1 medications—produced by licensed pharmacies under FDA exceptions—became a lifeline for many patients. Traditional healthcare pathways often involved long wait times, insurance hurdles, and costs exceeding $1,000 monthly. Telehealth providers stepped in with streamlined, digitally native experiences.
Trimi, recognized as one of Forbes Health's Best GLP-1 Medications of 2026, exemplifies this direct-to-consumer approach. Founded as a specialized telehealth platform for GLP-1 treatments, Trimi combines licensed clinical oversight with optimized online workflows for consultation, prescription, and fulfillment. Its model emphasizes affordability and simplicity: compounded semaglutide and tirzepatide offered at consistent pricing starting under $150 per month, with some plans as low as $99–$125, eliminating dose-dependent price jumps common elsewhere.
By operating lean—without heavy physical infrastructure—and focusing on user experience, Trimi scaled efficiently while addressing key barriers. The platform's infrastructure also positions it for potential expansion into adjacent areas like hormone therapy and chronic care.
The Regulatory Reckoning
As supply chains matured, the regulatory landscape shifted dramatically. The FDA declared the shortages resolved—tirzepatide in late 2024 and semaglutide in February 2025—removing the primary legal foundation for large-scale compounding. Phased enforcement deadlines followed for 503A pharmacies and 503B outsourcing facilities. The agency also issued multiple waves of warning letters, totaling dozens, to telehealth companies, citing false or misleading claims that suggested compounded products were equivalent to FDA-approved drugs in safety, efficacy, or quality.
Proposals were advanced to exclude semaglutide, tirzepatide, and liraglutide from the 503B bulk drug substances list, further limiting large-scale production. Many smaller or less-prepared operators exited the market, scaled back services, or shut down entirely.
The transition carried real risks and disruptions. Patients who depended on affordable compounded GLP-1s often faced significantly higher costs when shifting to branded options, prescription changes, or interruptions in care continuity. Safety concerns also surfaced: the FDA has linked compounded semaglutide to adverse events, including dosing errors that led to hospitalizations and, in certain cases, more serious outcomes.
The Rise of Turnkey Telehealth Infrastructure
A parallel development reshaping the GLP-1 telehealth landscape is the proliferation of turnkey infrastructure companies that enable virtually anyone to launch a virtual clinic. These middlemen handle the operational heavy lifting—from finding rosters of doctors and nurses to ensuring regulatory compliance—allowing clients to purchase ready-built software and infrastructure and get started quickly.
Fuse Health, a newer turnkey startup specializing in peptides, boasts that it can help brands go live in three hours. "Yes, we timed it," its website reads. CareValidate, another leading turnkey firm, launches at minimum one new brand per day, according to chief medical officer Jiten Chhabra, who describes this as an era of "telehealth for everything."
The digital search company JustAnswer made an odd pivot into selling weight loss drugs, using a company called WhiteLabelMD to handle customer service, provide software, and connect patients with clinicians. "We wouldn't have gotten into this if we had to build it all from scratch," said chief executive Andy Kurtzig. The gay dating app Grindr launched a telehealth arm, Woodwork, in 2025, using OpenLoop to supply erectile dysfunction medication, the peptide Sermorelin, and GLP-1 drugs.
"It's going to change the game," said Scott Roth, chief executive of LegitScript, a company that provides certifications for telehealth providers and pharmacies. "Anyone who has an audience could potentially get into this—a gym, an influencer, you name it."
The GLP-1 clinic Medvi used turnkey companies to propel itself to over $400 million in sales in 2025 without ever hiring anyone who worked in health care. A February FDA warning letter was addressed to the company, and it was the subject of an April class action lawsuit. Reality television star Savannah Chrisley started Good Girl RX in 2025; it also received an FDA warning letter for allegedly making "false or misleading" claims about its products.
Sabina Hemmi, cofounder and chief executive of the GLP-1 provider comparison startup GLP Winner, said she no longer attempts to track new startups because there are simply too many. "We're seeing a lot of vibe-coded sites," she said. "There's been a rise of what I call 'marketing bros' getting into the space." She characterized the phenomenon as the "Temu experience of telehealth."
What Separates Survivors
Platforms that thrived invested in robust infrastructure. Key differentiators included HIPAA-compliant tech stacks, strong electronic health record integrations, secure pharmacy partnerships, and transparent clinical oversight.
Sohail Aslam, CEO of Staunch—the engineering backbone for multiple high-growth GLP-1 telehealth providers collectively serving over 100,000 patients annually—provided insight into what made the difference. "Providers who had diversified beyond compounded weight loss into adjacent wellness categories like hair loss and ED, or had already begun offering branded options, were simply better positioned to absorb the shock," Aslam said. He stressed that scalable, compliant technology enabling legitimate clinical workflows and quick pivots proved essential for survival and continued growth.
The industry is now shifting toward branded partnerships. Companies like Ro, Hims & Hers, and LifeMD have collaborated with manufacturers to distribute FDA-approved Wegovy, Zepbound, and similar products, often with cash-pay options and streamlined access. Emerging oral formulations and expanded indications further support long-term market growth, projected toward $100 billion-plus by the mid-2030s.
Patient Impact and Quality Concerns
For patients, the changes brought both progress and challenges. Lower-cost access during the boom helped thousands manage weight and related conditions, but the reckoning highlighted risks around quality consistency, supply stability, and treatment continuity.
There is a wide spectrum in quality for virtual clinics, and it can be difficult for consumers to tell which ones provide robust support. Hemmi noted that two different clinics will offer "nutrition support," but one will offer unlimited phone calls with registered dieticians while the other will simply email "an AI slop PDF" on what to eat. "To me, those are two completely different standards of care," she said.
For consumers seeking recourse when things go wrong, clinics using turnkeys may not provide answers because they do not actually employ the prescribing clinicians. Ateev Mehrotra, chair of health services, policy, and practice at Brown University, noted that a person would typically sue the clinician rather than the clinic. "It's on the clinician, but I wouldn't be surprised in the coming years, because they're pushing the boundaries, that lawsuits might actually go after the companies—but we haven't seen it so far," he said.
Using a turnkey service does not necessarily connote a lack of quality—many trustworthy, thoughtfully designed telehealth platforms use them as well. Roth said that while turnkeys can draw in people without any experience in health care, these companies also provide a scaffolding to help clients "do it the right way."
The GLP-1 story illustrates a classic healthcare pattern: clinical innovation races ahead, while distribution, pricing, and regulation must catch up. Digital telehealth platforms built for speed, affordability, and scalability remain essential. Those that combine strong technology, clinical rigor, and adaptability are best equipped to deliver equitable access in a maturing market.
