The research, led by Carly Sillcox from McGill University Health Centre, highlights the importance of challenge tests in confirming macrolide allergies due to the lack of standardized skin tests.
Evaluating Macrolide Allergy in Children
Macrolides (search) are frequently prescribed as alternatives to β-lactams for individuals with allergies, accounting for up to 30% of all oral antibiotic prescriptions. However, suspected allergic reactions can lead to lifelong avoidance of these drugs, often without specialist assessment. The study aimed to evaluate the safety of direct GOCs in confirming suspected macrolide allergies in children.
Study Design and Methodology
The study, conducted between November 2012 and November 2023, included children aged 18 or younger with suspected macrolide allergies from allergy clinics in Montreal and Newfoundland and Labrador. Participants underwent a two-step, non-blinded GOC involving a 10% dose followed by a 90% dose of the suspected macrolide allergen, with observation for one hour. Families were instructed to report any delayed symptoms within a week.
Data collected retrospectively included demographics, clinical details of the suspected allergic reactions, and their management, using a standardized questionnaire. Multivariate logistic regression analysis was used to identify factors associated with positive GOC reactions, with a significance level set at P < 0.05.
Key Findings
Of the 112 participants, 58.9% were male, and the median age was 5.5 years. The implicated macrolides (search) were clarithromycin (72.3%) and azithromycin (27.7%). The study found that 90.2% of direct GOCs were tolerated, while 8.0% resulted in immediate reactions and 1.8% in non-immediate reactions. The majority of positive GOC findings involved skin-limited rashes, with only one subject experiencing anaphylaxis (search).
Children older than 12 years were more likely to have positive GOC results (adjusted odds ratio [AOR], 1.43; 95% CI, 1.26-1.61), as were those with other antibiotic allergies (AOR, 1.22; 95% CI, 1.02-1.46). Conversely, reactions were less likely among subjects with known food allergies (AOR, 0.84; 95% CI, 0.73-0.97).
Clinical Implications
The study's findings suggest that direct GOCs are a valuable tool for accurately diagnosing macrolide allergies in children, potentially preventing unnecessary avoidance of these important antibiotics. Follow-up interactions revealed that 17.4% of those with negative GOC results had successfully used their culprit macrolide without adverse reactions.
Limitations
The researchers acknowledged recall bias as a limitation due to the method of documenting index reaction characteristics, though the time between the index macrolide reaction and GOC was under a year, which likely limits the implications of recall bias.
