The study represents one of the largest datasets available for evaluating reduced-dose chemotherapy (search) approaches in elderly lymphoma (search) patients.
Validating Reduced-Dose Treatment in Community Settings
While previous research had established the general effectiveness of lower chemotherapy (search) doses for elderly patients, this study specifically evaluated whether the approach remained valid in "real world" clinical settings outside of National Cancer Institute-designated academic centers. The findings confirm the safety and effectiveness of reduced-dose treatment for millions of people seeking care in non-academic, community cancer (search) clinics.
Wallace and Barr partnered with COTA Healthcare (search), a company that collects data for cancer (search) research, to chart outcomes for patients who received "mini-R-CHOP (search)" instead of the standard R-CHOP (search) chemotherapy (search) cocktail typically used to treat DLBCL (search). The mini dose achieved the same cure rates as full-dose treatment.
"It also seemed like people who got mini-R-CHOP (search) had a lower rate of stopping treatment because of side effects compared to full-dose R-CHOP (search)," Wallace explained. "We want people to get all their planned chemotherapy (search). In this study, it looks like a Goldilocks regimen—where it's preserving effectiveness but limiting toxicity."
Advancing Treatment Optimization for Elderly Patients
The research supports the principle that elderly cancer (search) patients should not receive the same chemotherapy (search) doses as younger individuals. As the researchers noted, it doesn't make sense to give an 80-year-old cancer patient the same dose of chemotherapy as a younger person, and research has already proven this general approach is effective.
Building on these findings, Wallace is currently enrolling older patients with DLBCL (search) into a new clinical trial evaluating mini-R-CHOP (search) combined with the immunotherapy drug mosunetzumab (search). This study represents the third in a series of trials focused on refining treatments for the oldest patients and is supported by the Lymphoma (search) Scientific Research Mentoring Program.
The foundation for Wallace's current trial stems from previous studies testing mosunetzumab (search) in lymphoma (search) patients conducted by Dr. Patrick Reagan, associate professor of medicine at the University of Rochester.
Addressing the Growing Need for Age-Appropriate Cancer Care
The work aligns with Wilmot Cancer Institute (search)'s primary goal of addressing all aspects of aging and cancer (search), particularly important given the region's large and growing population of people ages 65 and older. Dr. Jonathan Friedberg, Wilmot Director, emphasized the significance of the research approach.
"It's important to emphasize that our investigators are not only working hard to eliminate or reduce side effects of cancer (search) treatment, particularly for our older patients—but they are also leading and initiating clinical trials," Friedberg said. "Providing access to cutting-edge studies and offering treatments that are not yet widely available is a primary benefit of being an NCI-designated cancer center."
