Home-Based Gamma Brain Stimulation Shows Promise for Early Alzheimer's Disease in Randomized Trial

A randomized clinical trial published in JAMA Network Open has demonstrated that home-based gamma transcranial alternating current stimulation (search)Search drug (tACS (search)Search drug) can significantly improve cognitive function and daily living activities in patients with early Alzheimer's disease (search)Search disease. The study, conducted at a tertiary Alzheimer research clinic in Italy between December 2022 and October 2024, represents the first rigorous evaluation of portable brain stimulation technology delivered outside clinical settings for this patient population.

Trial Design and Patient Population

Safety and Feasibility Outcomes

No serious adverse events were reported throughout the study period, and adherence was exceptionally high with fewer than 2% of sessions missed. The most commonly reported adverse effect was transient light flickering, which did not interfere with patient participation. Importantly, caregiver burden related to home treatment was reported as low, supporting the feasibility of this approach in real-world settings.

Clinical Efficacy Results

During the randomized phase, participants receiving active stimulation showed statistically significant improvements compared with the sham group across multiple clinical measures. Neurophysiologic Evidence of Brain Engagement

Neurophysiologic testing provided compelling evidence of biologic engagement with the targeted brain networks. Electroencephalographic (EEG) measurements showed increased gamma-frequency activity over parietal and frontal regions following gamma tACS (search)Search drug, consistent with targeted modulation of brain oscillations. These changes were not observed during sham treatment.

Measures of cholinergic neurotransmission (search)Search target, assessed using transcranial magnetic stimulation, also improved during active stimulation and correlated with improvements in associative memory performance. Amyloid-beta (search)Search target, phosphorylated tau217 (search)Search target, neurofilament light, and glial fibrillary acidic protein did not change significantly during the study period. Measures of functional brain connectivity on magnetic resonance imaging also remained stable over the treatment period.

Treatment Duration and Optimization

The investigators found no added benefit from extending treatment beyond eight weeks, as clinical and neurophysiologic measures plateaued with longer stimulation. This finding suggests an optimal treatment duration that balances efficacy with practical considerations for patients and caregivers.

Clinical Implications and Future Directions

The researchers emphasized that while blood-based biomarkers did not reflect short-term changes, neurophysiologic measures provided clear evidence of biologic engagement with disease-relevant brain networks. The findings support the potential for gamma tACS (search)Search drug to become a practical adjunctive therapy for early Alzheimer's disease (search)Search disease, particularly given its home-based delivery and favorable safety profile.

However, the investigators noted that larger trials with longer follow-up will be needed to determine the durability of these effects and whether biomarker changes emerge over time.
