The results, published in JAMA Surgery, demonstrate the agent's potential to address a significant unmet medical need in surgical nerve visualization.
Novel Mechanism Targets Myelin for Enhanced Visualization
Illuminare-1 represents a breakthrough approach to intraoperative nerve imaging as a small-molecule fluorophore that binds specifically to myelin, the nerve sheath substance. Unlike other commonly used intraoperative dyes that do not directly bind to tissue, this agent fluoresces under blue light to selectively illuminate nerves during surgery, helping surgeons avoid unintended nerve injury.
"Seeing nerves clearly during surgery remains a major challenge," said Timothy Donahue, M.D., urologic surgeon at Memorial Sloan Kettering Cancer Center (MSK) and principal investigator of the Phase 1 study. "Further testing will show whether this technology can improve prostate cancer surgery — and possibly benefit other procedures too."
Phase 1 Trial Demonstrates Dose-Dependent Efficacy
The single-arm, open-label Phase 1 study was conducted at MSK between January 2023 and October 2024, enrolling 38 patients undergoing robot-assisted laparoscopic radical prostatectomy (RALP). The trial aimed to determine the optimal safe and clinically effective dose of Illuminare-1 for sustained intraoperative nerve visualization, using the obturator nerve (search) as the reference structure to measure fluorescence intensity.
Study findings revealed that a 3.0 mg/kg dose was well tolerated and clinically effective for sustained fluorescence of nerve structures during RALP surgery. The probability of achieving moderate or better subjective fluorescence was highly correlated with increased Illuminare-1 dose, with dose-dependent efficacy demonstrated across sequential dose escalation cohorts.
Strong Clinical Outcomes in Critical Nerve Structures
In the expansion cohort receiving the effective dose, the clinically relevant peri-prostatic neurovascular bundles (search) that control erectile function demonstrated fluorescence in 8 of 9 patients (89%). These small bundles are typically difficult to observe during surgery, making their clear visualization particularly significant for preserving patient function post-operatively.
The agent showed rapid onset of action, with nerves fluorescing within 15 minutes of intravenous injection at the effective dose. Sustained fluorescence was achieved in all patients at the effective dose, with the obturator nerve (search) showing fluorescence for 90 minutes or longer, generally still fluorescing at wound closure.
Excellent Safety Profile Across All Doses
Throughout the dose escalation study, Illuminare-1 maintained a clean safety profile at all tested doses, with the study concluding that the agent was well tolerated across the patient population. This safety profile supports the potential for broader clinical application and development.
Broader Surgical Applications on the Horizon
According to the concurrently published invited Commentary in JAMA Surgery by Atkinson and Sheu, the trial represents a "pivotal step toward enhancing intraoperative nerve visualization." The commentary authors suggested that Illuminare-1's application could extend to many other procedures, including inguinal hernia repairs, thyroidectomies, and axillary dissections.
