Vancomycin Therapeutic Drug Monitoring Reduces Mortality in Sepsis Patients

Vancomycin therapeutic drug monitoring (TDM) is associated with a significant reduction in 30-day mortality among sepsis (search) patients, according to a recent study published in Frontiers in Medicine. The retrospective cohort study, which analyzed data from the MIMIC-IV database, highlights the potential benefits of personalized vancomycin dosing in improving patient outcomes. Sepsis, a life-threatening condition caused by the body's overwhelming response to an infection, is a major global health concern, and vancomycin is a commonly used antibiotic in its treatment.
The study, which examined data from 14,053 sepsis (search) patients admitted to the intensive care unit (ICU) and treated with vancomycin, found that patients who underwent TDM had a lower 30-day mortality rate compared to those who did not. Before propensity score matching (PSM), the 30-day mortality rate was 27.2% in the TDM group and 16.2% in the non-TDM group. After PSM, analysis of 4,329 matched pairs revealed a significantly lower 30-day mortality in the TDM group (23.3%) compared with the non-TDM group (27.7%) (p < 0.001).
Impact of TDM on Mortality Risk
Multivariable Cox proportional hazards regression demonstrated a significantly reduced 30-day mortality risk in the TDM group (adjusted hazard ratio [HR]: 0.66; 95% confidence interval [CI]: 0.61-0.71; p < 0.001). This finding was further supported by PSM-adjusted analysis (adjusted HR: 0.71; 95% CI: 0.66-0.77; p < 0.001) and inverse probability of treatment weighting (IPTW) analysis (adjusted HR: 0.72; 95% CI: 0.67-0.77; p < 0.001). Kaplan-Meier survival curves also indicated significantly higher 30-day survival in the TDM group (log-rank test, p < 0.0001).
Age-Related Differences
Interestingly, the study also found that the benefits of vancomycin TDM were most pronounced in sepsis (search) patients aged 18 to 50 years. This subgroup exhibited the lowest mortality rates and the greatest reduction following TDM compared with older patients. This suggests that age may play a role in the effectiveness of vancomycin TDM in sepsis patients.
