Inflammasome Therapeutics' K8 Implant Reduces Geographic Atrophy Lesion Growth by 53% in Phase I Trial

Inflammasome Therapeutics (搜索) announced positive results from its first-in-human clinical trial of K8, a sustained-release intraocular implant that reduced geographic atrophy (搜索) (GA) lesion growth by 53% after three months of treatment. The multicenter Phase I trial represents a significant advancement in treating this progressive retinal disease that affects approximately one million people in the United States and more than eight million worldwide.
Trial Design and Patient Population
The ongoing multicenter trial (NCT06164587) is evaluating K8 in 30 patients with bilateral GA across three dose-escalation cohorts over six months. Patients receive low, medium, or high-dose implants at baseline and three months in one eye, with untreated fellow eyes serving as controls. The initial results come from the low-dose cohort of 10 treated eyes.
"Patients receiving the three-month, low dose, implant showed an average 53% reduction in lesion growth compared to untreated eyes at three months after a single injection," said Paul Ashton, PhD, CEO and co-founder of Inflammasome Therapeutics (搜索). Despite the small sample size, the trial achieved statistical significance with a p-value of 0.03 using a mixed-effect model.
Visual Acuity Outcomes
Beyond lesion growth reduction, K8 demonstrated positive effects on visual function. Best corrected visual acuity increased an average of 1.4 ETDRS letters in K8-treated eyes, while untreated eyes deteriorated, losing an average of 1.9 ETDRS letters over the three-month period.
"Vision trends were also positive: eyes receiving the K8 implant had essentially stable vision," Ashton noted. "GA is a slowly progressive disease and over time we would expect vision in untreated eyes to continue to deteriorate."
Mechanism of Action and Competitive Landscape
K8 belongs to a new class of drugs called Kamuvudines, which are derivatives of anti-HIV drugs (NRTIs) designed to inhibit multiple inflammasomes while maintaining improved safety profiles. This broad mechanism distinguishes K8 from currently approved treatments.
"GA is a multifactorial disease in which many inflammatory toxic factors such as various forms of complement (搜索), amyloid beta, retrotransposons, oxidative stress are upregulated in the diseased eye, which causes upregulation of the inflammasome (搜索) pathway and atrophy of cells in the macula," explained Jayakrishna Ambati, MD, co-founder of Inflammasome Therapeutics (搜索).
The two FDA-approved drugs for GA, SYFOVRE® and IZERVAY™, each target different forms of complement (搜索) and reduced lesion growth by approximately 20% with monthly intraocular injections in Phase III trials, but did not reduce vision loss over 12 months.
Safety Profile and Trial Progression
The trial reported no adverse safety signals in the initial cohort. The company has completed enrollment in the mid-dose cohort and is currently enrolling patients in the high-dose cohort to complete the dose-escalation study.
Broader Pipeline Applications
Inflammasome (搜索) activation may play key roles in multiple neuroinflammatory and ocular diseases, including Alzheimer's disease (搜索), ALS (搜索), Parkinson's disease (搜索), multiple sclerosis (搜索), and diabetic macular edema (搜索). The company has initiated two additional Phase I/II trials with a second Kamuvudine (搜索), K9, delivered orally for diabetic macular edema (NCT06781255) and thyroid eye disease (搜索) (NCT06467435).
"We believe the impressive result seen in this clinical trial and in numerous preclinical studies are due to the unique mechanism of K8, which inhibits activation of multiple inflammasomes and thereby blocks the effects of numerous inflammatory toxins, including complement (搜索)," Ambati said.
