Promising Preclinical Results Drive Trial Selection
The selection of Inflammasome Therapeutics (search) for the ALS (search) MyMatch program was based on compelling preclinical data demonstrating K9's potential therapeutic effects. According to Dr. Jayakrishna Ambati, co-founder of Inflammasome Therapeutics, "Preclinical studies of K9, our lead drug for ALS, have been extremely encouraging, showing a profound effect in reducing a biomarker that is prognostic of disease progression."
Specifically, K9 reduces levels of neurofilament light (NfL (search)) chain in human ALS (search) motor neurons and in animal models by 80% or more. This biomarker reduction is particularly significant given that NfL is released by dying neurons and is elevated 10-times higher in individuals with ALS compared to healthy individuals.
Addressing Unmet Medical Need in ALS Treatment
The collaboration addresses a critical gap in ALS (search) treatment options. Dr. Ambati noted that while Qalsody was approved by the FDA based on its ability to reduce blood levels of NfL (search) by about 55%, this treatment is indicated only for patients with a specific genetic mutation. "Unfortunately, about 95% of individuals with ALS don't qualify for this treatment," he explained.
K9 represents a first-in-class multi-inflammasome inhibitor that targets neuroinflammation, a disease process that characterizes not only ALS (search) but also multiple sclerosis (search), Parkinson's disease (search), Alzheimer's disease (search), and macular degeneration (search). Dr. Paul Ashton, co-founder and CEO of Inflammasome Therapeutics (search), emphasized that "activation of inflammasomes (search) causes neuroinflammation, which, in turn, is associated with diseases that involve neuronal cell death."
Innovative Trial Design Through ALS MyMatch
The Healey ALS (search) MyMatch program represents a novel approach to ALS clinical trials, integrating comprehensive genetic and biofluid biomarkers to match subgroups of ALS individuals to various experimental therapies based on their disease markers. The program consists of ongoing early-phase clinical trials (Phase 1b/2a) aimed at deepening understanding of biological effects of experimental products and identifying optimal populations for future Phase 2/3 clinical trials.
Dr. Merit Cudkowicz, Director of the Healey & AMG Center, highlighted the importance of this approach: "Early phase discovery trials are critical to understand dosing, target engagement and population prior to embarking on efficacy trials. This is an area that has not been funded sufficiently; and where therapy development often slows down tremendously."
Supporting Clinical Evidence
The confidence in K9's potential extends beyond ALS (search) applications. The drug is related to K8, which is currently being tested in a Phase 2 clinical trial for macular degeneration (search), another neuroinflammatory disease. Dr. Ambati noted that "early data from this trial are very promising, giving us further confidence in our ALS program."
Multi-Site Collaborative Infrastructure
ALS (search) MyMatch operates as a multi-site collaborative initiative, currently bringing together four trial-ready, high-enrolling ALS research centers as part of the Network of Excellence for ALS (NEALS) Consortium affiliated program. The participating research centers include Massachusetts General Hospital in Boston, University of Minnesota in Minneapolis, Northwestern University in Evanston, and Nova Southeastern University in Fort Lauderdale.
