Infliximab Biosimilar CT-P13 Demonstrates Safety and Efficacy in Real-World Rheumatoid Arthritis Treatment

A recent prospective clinical trial has demonstrated that patients with rheumatoid arthritis (search) (RA) in clinical remission can safely switch from originator infliximab to the biosimilar CT-P13 without experiencing an increased risk of disease relapse. The findings, published in Drug Discoveries & Therapeutics, provide crucial evidence supporting the use of cost-effective biosimilar alternatives in RA treatment.
Clinical Trial Results Show Low Relapse Rates
The IFX-SIRIUS study I, a 24-week prospective, open-label, interventional, single-arm clinical trial, enrolled 18 patients with RA who had achieved clinical remission during treatment with originator infliximab (Remicade; Janssen Biotech (search), Inc.). Patients were switched to infliximab-dyyb (CT-P13, Remsima, Inflectra; Celltrion) using an unchanged dosing regimen.
From baseline to week 24, approximately 11.1% (n = 2) of patients experienced clinical relapse (95% CI 3.1–32.8). Importantly, there were no changes observed in musculoskeletal ultrasound (MSUS) scores, including total grayscale and power Doppler scores, Disease Activity Score 28 (DAS28)-erythrocyte sedimentation rate, DAS28-C-reactive protein, Health Assessment Questionnaire-Disability Index, and van der Heijde-modified total Sharp score from baseline to week 24.
The study utilized comprehensive assessment methods including clinical disease activity indices, MSUS, and serum cytokines/chemokines. Serum levels of multiple cytokines and chemokines showed no apparent changes throughout the study period.
Safety Profile Remains Favorable
The safety profile of CT-P13 proved reassuring, with only three nonserious adverse events occurring during the study period. Crucially, there were no study discontinuations as a result of adverse events, indicating good tolerability of the biosimilar switch.

Real-World Evidence Supports Clinical Findings

The ReFLECT study addressed skepticism regarding biosimilar clinical equivalence by providing compelling evidence of CT-P13's effectiveness. Early results demonstrated that patients receiving the biosimilar maintained similar clinical outcomes to those treated with the original drug, with adverse events reported at rates comparable to those observed with original infliximab.
Background on Infliximab and CT-P13
Infliximab is an FDA-approved monoclonal antibody that targets tumor necrosis factor-alpha (search) (TNF-α (search)) to address inflammatory conditions, including Crohn disease (search), ulcerative colitis (search), RA, ankylosing spondylitis (search), psoriatic arthritis (search), and severe plaque psoriasis (search). CT-P13's initial approval was based on results from two clinical trials demonstrating equivalence to reference infliximab in terms of pharmacokinetic properties in patients with ankylosing spondylitis (search) and efficacy in patients with RA.
