Iptacopan Shows Promise in Treating Life-Threatening Transplant-Associated Thrombotic Microangiopathy

A 17-year-old patient with life-threatening transplant-associated thrombotic microangiopathy (search) (TA-TMA (search)) has achieved remarkable recovery following treatment with iptacopan, an oral complement factor B (search) inhibitor, according to a case report published in Frontiers in Immunology. The successful outcome represents a potential breakthrough for treating this devastating complication of hematopoietic stem cell transplantation, which carries mortality rates of 50% to 90%.
Critical Condition Following Stem Cell Transplant
The patient, diagnosed with high-risk myelodysplastic syndrome (search), underwent HLA-matched sibling donor allogeneic peripheral blood stem cell transplantation in July 2024. Thirty-five days post-transplantation, he developed severe complications including abdominal pain, diarrhea, reduced urine output, and diminished mental state.
Laboratory findings revealed the hallmarks of TA-TMA (search): platelet count dropped to 27 × 10⁹/L, lactate dehydrogenase elevated to 615 U/L, and C5b-9 levels reached 739 ng/mL (reference range: 75-219 ng/mL). The patient also developed multi-organ dysfunction with acute renal failure (creatinine 592.5 μmol/L), acute liver failure (ALT 1379 U/L, AST 990 U/L), and pulmonary infection.
"Given that TA-TMA (search) is characterized by an elevated ACR (> 2mg/mg), significantly elevated C5b-9 (above the upper reference limit), and multi-organ dysfunction, the patient was diagnosed with TA-TMA according to the above criteria," the researchers reported.
Conventional Treatment Limitations
Initial treatment followed standard protocols with plasma exchange, daily hemodialysis, and a single dose of eculizumab (900 mg). However, the patient's critical condition prompted physicians to switch to iptacopan after five days of conventional therapy.
The case highlights a key limitation of eculizumab, the current standard C5 (search) inhibitor. While eculizumab can rapidly control intravascular hemolysis by targeting the terminal complement pathway, it cannot suppress the amplification loop of the alternative complement pathway. This results in incomplete hemolysis control and can trigger extravascular hemolysis as C3b (search) deposits on red blood cell surfaces.
Breakthrough with Proximal Complement Inhibition
Iptacopan works differently by targeting factor B (search), a central regulatory protein in the alternative complement pathway. The drug "binds directly and selectively to factor B, blocking its cleavage by factor D and thereby inhibiting the generation of Ba and Bb fragments," preventing the formation of C3bBb and halting the activation and amplification cycle.
The patient received iptacopan at 200 mg twice daily for one month, during which his condition significantly improved. After 10 weeks of treatment, key biomarkers showed dramatic improvement:
- C5b-9 levels dropped from 739 ng/mL to 163 ng/mL
- Platelet count recovered from 27 × 10⁹/L to 146 × 10⁹/L
- LDH decreased from 615 U/L to 279 U/L
- Urinary albumin/creatinine ratio fell from 342.277 mg/g to 29.78 mg/g
- Haptoglobin recovered from <0.06 g/L to 0.38 g/L
Managing Concurrent Infections
The treatment was complicated by multiple infections requiring targeted therapy. Epstein-Barr virus DNA was detected and treated with rituximab administered weekly for three doses. Cytomegalovirus DNA was identified and treated with cidofovir, while Klebsiella pneumoniae was managed with ceftriaxone based on sensitivity testing. All infections were successfully controlled.
Clinical Significance and Future Implications
TA-TMA (search) represents one of the most serious complications following hematopoietic stem cell transplantation, with the pathophysiology involving complex interactions between endothelial cell activation, complement cascade activation, and microthrombus formation. The condition follows the "Three-Hit Hypothesis," where patients with underlying complement predisposition or endothelial injury undergo toxic conditioning regimens, followed by additional insults such as medications, infections, or antibodies.
The alternative complement pathway plays a significant role in TA-TMA (search) pathogenesis, with several studies showing that Ba levels significantly increase in early stages following allogeneic HSCT and are closely associated with TA-TMA onset.
"This patient was classified as a high-risk TA-TMA (search) patient, with an extremely high mortality rate," the researchers noted. The successful outcome with iptacopan is particularly significant given that conventional first-line treatments focus on identifying and removing triggering causes and providing supportive care, while second-line treatments like plasma exchange, eculizumab, rituximab, and defibrotide are used in combination when first-line therapy fails.
Advantages of Oral Administration
Beyond its mechanism of action, iptacopan offers practical advantages as an oral formulation. The researchers noted that "as an oral formulation, iptacopan is more convenient and better tolerated than the intravenous eculizumab."
While iptacopan has shown promise in treating atypical hemolytic uremic syndrome and paroxysmal nocturnal hemoglobinuria, its application in TA-TMA (search) remains underexplored. The case authors concluded that "this case report demonstrates the potential efficacy of iptacopan in the treatment of high-risk TA-TMA, particularly in patients with multi-organ dysfunction and severe complement activation."
The researchers emphasized that further investigation, including larger, well-designed clinical trials, is needed to validate these findings and establish iptacopan as a standard treatment option for TA-TMA (search). However, this case provides compelling evidence that targeting the proximal complement pathway may offer a novel therapeutic approach that overcomes the limitations of existing C5 (search) inhibitors.
