IQIRVO Shows Promising Real-World Results for Primary Biliary Cholangitis at EASL 2026

Ipsen announced compelling new data for IQIRVO (elafibranor) at the European Association for the Study of Liver Disease (EASL) 2026 congress, reinforcing the drug's position as the only second-line primary biliary cholangitis (search) (PBC (search)) treatment demonstrating improvements in both biochemical markers and debilitating symptoms including fatigue and pruritus.
Breakthrough Results in Fatigue Management
A post-hoc analysis of the pivotal ELATIVE Phase III trial revealed significant improvements in fatigue among PBC (search) patients. Those with moderate-to-severe fatigue at baseline experienced greater reductions with IQIRVO compared to placebo across 52 weeks, with 67% achieving clinically meaningful improvement versus 31% on placebo (p=0.020) at Week 52. Improvements were observed as early as Week 4.
"Fatigue is one of the most debilitating symptoms of PBC (search) and has long represented a significant unmet need for patients and clinicians," said David Jones, Professor of Liver Immunology for the Faculty of Medical Science at Newcastle University. "These data are particularly encouraging, showing that IQIRVO led to clinically meaningful improvements in fatigue compared with placebo."
The benefits extended across multiple dimensions of physical and mental fatigue, including extreme exhaustion (62% vs 31%), cognitive impairment manifesting as being too tired to think clearly (57% vs 31%), and functional limitations such as being too tired to bathe or shower (55% vs 25%).
Real-World Evidence in Mildly Elevated ALP Patients
In a first-of-its-kind real-world analysis using the Health Verity database, researchers evaluated outcomes in patients with alkaline phosphatase (ALP (search)) elevated between 1-1.67 times the upper limit of normal (ULN). The study demonstrated significant improvements through month 6 in treatment-naïve patients, with 72% achieving ≥15% ALP reduction and mean ALP declining from 174 U/L to 131 U/L. Notably, 59% of patients achieved ALP normalization, a key treatment goal associated with improved long-term prognosis and slowed disease progression.
ELFINITY Phase IV Study Confirms Clinical Benefits
Interim findings from the global Phase IV ELFINITY study, conducted in routine clinical practice, showed IQIRVO delivered rapid and sustained ALP (search) reductions over three months, with biochemical response achieved in 55% of patients. More than half of patients with baseline moderate-to-severe fatigue experienced clinically meaningful improvements by month 3, alongside clinically important improvements in pruritus. The drug demonstrated a favorable tolerability profile consistent with previous studies, with no serious or severe treatment-emergent adverse events reported.
Clinical Significance and Mechanism of Action
IQIRVO represents the first approved peroxisome proliferator-activated receptor (PPAR (search)) agonist with dual α and δ activation. The drug's mechanism involves decreasing bile toxicity and improving cholestasis by modulating bile acid synthesis, detoxification and transporters, while providing complementary anti-inflammatory, anti-cholestatic, anti-fibrotic and metabolic effects.
"The growing evidence from both controlled trials and real-world practice highlights the meaningful difference IQIRVO can make for people living with PBC (search)," said Sandra Silvestri, MD, PhD, Chief Medical Officer at Ipsen. "Across multiple studies, we see consistent improvements in both biochemical control, a key marker of disease progression, and in the debilitating symptoms of fatigue and pruritus, which are independent of each other."
Regulatory Status and Market Position
IQIRVO received U.S. FDA accelerated approval in June 2024, conditional approval by the EMA in September 2024, and UK MHRA approval in October 2024 for treating PBC (search) in combination with ursodeoxycholic acid (UDCA) in adults with inadequate response to UDCA, or as monotherapy in patients unable to tolerate UDCA. The drug is also approved in Canada, Australia, Brazil and 13 other countries, with regulatory processes ongoing in additional territories.
Primary biliary cholangitis (search) affects approximately 100,000 people in the US and 115,000 people in Europe, predominantly women. The rare autoimmune liver disease involves bile and toxin buildup with chronic inflammation causing irreversible liver fibrosis and bile duct destruction, potentially leading to liver transplant or premature death if inadequately treated.
