Kidney Biopsies Reveal How SGLT2 Inhibitor Dapagliflozin Protects Against Early Diabetic Kidney Disease in Youth with Type 1 Diabetes

A groundbreaking study published July 22 in Science Translational Medicine has provided the first direct molecular evidence from human kidney biopsies showing how the SGLT2 (search) inhibitor dapagliflozin protects the kidneys of adolescents and young adults with type 1 diabetes (search). The findings offer some of the strongest biological evidence to date for the kidney-protective effects of this drug class and raise the possibility of shifting clinical strategy from repairing established kidney damage to preventing it altogether.
"This was a look 'under the hood' at what an SGLT2 (search) inhibitor does inside the kidneys," said corresponding author Dr. Petter Bjornstad, professor of pediatrics and medicine at the University of Washington School of Medicine (search) and executive director of the UW Medicine Diabetes Institute. "Most of what we know about these drugs comes from blood and urine tests, which tell you that the kidney is doing better but not why."
A window into early disease
Diabetic kidney disease (search) remains a leading cause of kidney failure worldwide, yet no new kidney-protective therapies have been approved for type 1 diabetes (search) in decades. Roughly one in three people with type 1 diabetes eventually develops chronic kidney disease, but the damage is typically undetected for a long time.
The randomized, placebo-controlled clinical trial enrolled 98 youth ages 12 to 21 with type 1 diabetes (search) and evaluated the effect of dapagliflozin, administered in combination with adjustable insulin, over a 16-week period. Critically, participants had no overt kidney damage but exhibited hyperfiltration — a subtle early warning sign in which the kidney is essentially straining and overworking.
"By the time standard tests turn abnormal, real damage has often already happened," Bjornstad said. "The window where you can prevent rather than repair is early — often in adolescence and young adulthood, long before anyone feels sick."
Single-cell mapping reveals whole-kidney response
The research team examined kidney tissue biopsies before and after treatment, analyzing the individual molecular signatures of more than 214,000 kidney cells. The scale of this single-cell analysis allowed researchers to map precisely which biological pathways the medication engaged.
The treated kidney cells demonstrated healthier energy use, less inflammation and oxidative stress, improved oxygen balance, and fewer signs of injury. More than 55% of the genes that changed with treatment shifted toward the patterns seen in healthy kidneys.
"We expected changes mostly in the sugar-handling cells where the drug directly acts, but we saw a coordinated, whole-kidney response reaching the blood vessel lining and the filter cells, too," Bjornstad said, describing the breadth of biological changes as surprising.
One initially concerning finding was that treated kidneys were less oxygen-rich. However, the researchers determined this was actually a positive sign. "Diabetic kidneys are abnormally over-oxygenated in a way that reflects inefficient, overworked plumbing, and the drug appears to bring that back toward the healthy pattern," Bjornstad explained. "It turned out to be a sign of the kidney working more efficiently."
Toward prevention rather than repair
The study's focus on young people early in their disease course yielded a particularly compelling insight: the kidney is remarkably "plastic," responding to the drug at a molecular level well before any standard test would flag a problem.
"That raises the real possibility of shifting from repairing kidney damage to preventing it. It still has to be proven in long-term studies, but the biology points toward 'the earlier, the better,'" Bjornstad said.
By parsing exactly which pathways the medication engaged, the findings may also aid future drug development. Bjornstad noted that future therapies could aim directly at those same pathways, potentially complementing SGLT2 (search) inhibitors or helping people who cannot take them due to contraindications.
Broader and larger trials are already underway to measure the effect of SGLT2 (search) inhibitors on kidney function in adults with type 1 diabetes (search) over years. The research was supported by funding from Breakthrough T1D (search).
