Landmark EPoCH Study Finds Socioeconomic Position Outweighs Parental Health Behaviors in Child Health Outcomes

A comprehensive new study published in PLOS Medicine (search)Search term challenges a central assumption of the Developmental Origins of Health and Disease (DOHaD) paradigm: that maternal health behaviors during pregnancy are the primary modifiable influences on child health. The Exploring Prenatal influences on Childhood Health (EPoCH) study, led by Sharp and colleagues, triangulated data from four longitudinal birth cohorts spanning two decades and found that socioeconomic position consistently outweighed individual parental behaviors in shaping childhood outcomes.

The study examined maternal and co-parent smoking, alcohol consumption, caffeine intake, and socioeconomic position in relation to more than 70 child health outcomes across early childhood. By combining conventional epidemiological meta-analyses with Mendelian randomization, negative control analyses, and genetic risk scores, the researchers moved beyond observational associations toward stronger causal inference.

"Across all models, effect estimates were generally small and P-values large," the authors reported. In the standard confounder-adjusted models, only 6% of results showed strong statistical evidence of larger-than-small associations before adjustment for the co-parent's exposure, dropping to 3% after mutual adjustment.

Maternal Smoking: The Notable Exception

Maternal smoking during pregnancy emerged as the primary health behavior with consistent evidence for causal effects. The study found five agreeing lines of evidence across analytical approaches for maternal smoking's effect on small for gestational age (search)Search disease at delivery, waist circumference at ages 5–7, BMI and overweight/obesity at ages 8–11, and multiple psychosocial outcomes including conduct problems, externalizing traits, hyperactivity/inattention, and depressive symptoms.

However, the associations between maternal smoking and body mass index were attenuated after adjustment for genetic risk scores, "suggesting that the observed associations may be partly explained by shared genetic influences," the authors noted.

Socioeconomic Position: The Dominant Factor

Low socioeconomic position demonstrated the most consistent associations across outcomes. In the fully adjusted model, 15% of results for low socioeconomic position showed strong statistical evidence of larger-than-small effects, compared to 6% for smoking, 3% for alcohol, and just 0.4% for caffeine.

"The widespread association we identified between low SEP and poorer offspring health outcomes agrees with the large body of research underscoring the significant impact of socioeconomic factors on child health, including adiposity and behavioural difficulties," the researchers wrote.

No Strong Evidence for Widespread Maternal Primacy

The study did not find widespread evidence of much stronger effects in mothers than partners. After mutual adjustment, 51% of analyses had a larger effect estimate for mothers than partners, and 52% had a smaller p-value for mothers. The percentage of results with strong evidence was 7% for mothers compared to 5% for partners.

Notably, partner alcohol consumption during pregnancy showed associations with psychosocial and cognitive outcomes, but the direction was often opposite to that seen for mothers. The partner alcohol genetic risk score analyses were mostly null, "indicating that this apparent protective effect could be explained by residual or unmeasured confounding."

Methodological Innovation and Open Science

The EPoCH study generated results from over 594,000 analyses involving parental exposures at up to nine timepoints, 72 outcomes over four age ranges, and eight adjustment sets. Sample sizes ranged from 35 to 232,139, with a mean of 18,554.

The researchers developed EPoCH Explorer, an RShiny web application that provides interactive and customizable data visualizations, verbal interpretations, and downloadable results. "By making their analytical framework and interactive EPoCH platform publicly available, they enable other researchers to interrogate the data, reproduce analyses, and generate new hypotheses," noted a perspective article accompanying the study.

Limitations and Generalizability

The authors acknowledged several limitations. Up to two-thirds of analyses were conducted within a single cohort due to differences in data availability. The predominance of white participants across cohorts—with the exception of Born in Bradford—limits assessment of how socioeconomic disadvantage intersects with ethnicity. All cohorts are based in high-income Northern European countries, constraining generalizability to other populations.

Some genetic risk scores were only weakly associated with the behaviors they were intended to represent, which may reduce the precision of genetically informed estimates. The study also focused on outcomes up to age 11 and did not examine later-life outcomes, miscarriage, or stillbirth.

Implications for Policy and Research

The findings carry significant implications for public health policy. "If the ambition is to improve child health and reduce inequalities, then interventions directed solely at changing maternal behaviors are unlikely to achieve substantial population-level gains," the accompanying perspective argued. "Instead, greater attention must be paid to the social and economic conditions in which pregnancies occur."

The study authors concluded that future DOHaD research "should avoid oversimplifying evidence on maternal pregnancy effects within a complex, socially patterned system, and instead broaden their scope to study data on partners, postnatal exposures, and wider social determinants of health."
