Traditional Metrics Underestimate Treatment Success

While WHO standard definitions classified merely 4.8% of Latvian patients as cured, long-term follow-up revealed that 76.9% of those affected remained permanently relapse-free, representing a 16-fold difference in perceived treatment success.
"The study underscores the importance of long-term follow-up in MDR-TB (search) and suggests that tuberculosis (search) control programmes should broaden their measures of success. Including recurrence-free survival rates allows for a more realistic assessment of the quality of care and the actual benefit to patients," says Sophie Meier, a medical PhD student at the Research Center Borstel (search), Leibniz Lung Center (search) (FZB) and the University of Lübeck (search).
Key Treatment Factors Identified
The researchers linked clinical data with national registry information for long-term follow-up, enabling the first systematic evaluation of long-term treatment outcomes in a former European country with high MDR-TB (search) incidence. A critical factor in treatment success was the use of at least three effective drugs in individual treatment regimens.
The analysis revealed important insights about treatment duration. Very short treatment courses of less than nine months were associated with increased risk of relapse or death. However, treatment durations between ten and seventeen months achieved comparable results to longer courses of treatment, suggesting an optimal therapeutic window.
Expert Panels Outperform Standard Guidelines
The study highlighted the superior performance of expert panels, known as consilia, in treatment selection and outcome assessment. In Latvia, the decisions made by the consilium were significantly superior to the results obtained by applying WHO definitions for MDR-TB treatment outcomes," explains PD Dr Thomas Brehm from the FZB and University Medical Center Hamburg-Eppendorf (search), the study's senior author.
These expert panels also serve as an element of effective antimicrobial stewardship against the development of new antibiotic resistance, according to the researchers.
Implications for Modern Treatment Approaches
The findings arrive at a time when MDR-TB (search) treatment has evolved significantly. After the study's observation period ended, MDR-TB treatments became more effective, with current treatment durations now aligned with the six months required for drug-sensitive tuberculosis (search).
The research provides important impetus for future treatment strategies for MDR-TB (search) and supports the use of individualized treatment regimens with sufficiently effective drugs. The authors note that prospective studies are now required to test these findings in the context of new, shortened treatment regimens using modern active substances.
