Legacy Healthcare Receives FDA Clearance to Advance Botanical Drug Cinainu into Phase 3 Trial for Alopecia Areata
核心洞察
Legacy Healthcare (搜索) announced FDA clearance for its botanical drug candidate Cinainu (搜索) to proceed with an international Phase 3 trial (RAAINBOW-2) for moderate to severe alopecia areata (搜索) in at least 500 subjects.
The approval follows successful Phase 2/3 results in pediatric populations published in the British Journal of Dermatology, where Cinainu (搜索) demonstrated clinically meaningful effect sizes with excellent safety profile.
Cinainu (搜索) represents the first topical treatment option for both moderate and severe alopecia areata (搜索) patients, offering an alternative to oral JAK inhibitors (搜索) with potential for sustained benefits after treatment discontinuation.
Legacy Healthcare (搜索) announced that the U.S. Food and Drug Administration has cleared the company's Investigational New Drug (IND) filing for Cinainu (搜索), allowing the botanical drug candidate to proceed with an international Phase 3 study in patients with moderate to severe alopecia areata (搜索). The multi-regional trial, designated RAAINBOW-2, will enroll at least 500 subjects and aims to support a New Drug Application.
Regulatory Milestone for Botanical Drug Development
The FDA clearance represents a significant regulatory milestone for Cinainu (搜索), which Legacy Healthcare (搜索) claims is the first multi-plant botanical drug candidate granted FDA clearance to advance into Phase 3 trials. According to Saad Harti, Founder and CEO of Legacy Healthcare, "FDA-regulated Botanical drug is an overlooked regulatory pathway, but the results of the RAAINBOW trial support that a botanical drug can treat complex chronic diseases without synthetic chemical intervention and no long-term health risks."
The approval follows successful completion of the international Phase 2/3 RAAINBOW study in children and adolescents with moderate-to-severe alopecia areata (搜索), which was published in the British Journal of Dermatology. The company has also secured agreement from Japan's Pharmaceuticals and Medical Devices Agency (PMDA) to conduct a global Phase 3 trial including Japanese adult and pediatric patients from age 2 to support registration in Japan.
Clinical Evidence and Safety Profile
In the initial RAAINBOW study conducted in pediatric populations with moderate to severe alopecia areata (搜索), Cinainu (搜索) demonstrated clinically meaningful effect sizes while maintaining an excellent safety profile and showing sustained benefits after treatment discontinuation. Over 700 patients have been exposed to Cinainu in clinical trials for up to 12 months, with no reported safety concerns.
Harti emphasized the unique characteristics of the treatment: "Beyond its efficacy, Cinainu (搜索) has shown to treat autoimmune AA without immunosuppression-related adverse events, and with a persistent effect after treatment discontinuation, both of which, to our knowledge, are firsts."
Mechanism of Action and Clinical Applications
In clinical and preclinical studies, Cinainu (搜索) demonstrated multiple mechanisms of action. The topical botanical drug candidate restored peri-follicular anti-apoptotic protein Bcl-2 (搜索) to near-normal levels, increased epidermal Langerhans cells density, and reduced endothelial expression of T-cell chemotaxin IL-8 (搜索) and pro-inflammatory adhesion molecules (E-selectin, ICAM-1). Additionally, the treatment increased scalp collagen content.
Clinical studies have shown positive effects on hair regrowth in patients with androgenetic alopecia (搜索), persistent chemotherapy-induced alopecia, and alopecia areata (搜索). The drug candidate maintains patent protection until 2043.
Addressing Unmet Medical Need
Dr. Arash Mostaghimi, who will lead the RAAINBOW-2 trial, highlighted the current treatment landscape and unmet need. As Vice Chair of Clinical Trials and Innovation at Brigham and Women's Hospital and Associate Professor at Harvard Medical School, Dr. Mostaghimi noted: "The approval of three oral JAK inhibitors (搜索) over the past 3 years has been a major breakthrough for the treatment of patients with severe AA. However, there still is a need for a safe, effective, and potentially remittive topical therapy for patients with AA across the spectrum of disease severity."
Cinainu (搜索) is positioned as the first topical drug candidate for both patients with severe and moderate alopecia areata (搜索), allowing for early treatment before disease progression.
Pipeline Expansion and Future Development
Legacy Healthcare (搜索) has initiated fundraising efforts to support the RAAINBOW-2 Phase 3 program and pipeline expansion. The company plans to conduct trials in atopic dermatitis (搜索) and psoriasis (搜索), where the product has shown efficacy signals. Additionally, an oral form of the product has demonstrated efficacy in cancer-related fatigue (搜索) in an NIH-sponsored study in mice.
The biopharmaceutical company focuses on leveraging the pleiotropic potential and safety of plant extracts into FDA-regulated botanical drugs, representing a novel approach to treating complex chronic diseases without synthetic chemical intervention.
