Lilly Launches Five-Year Real-World Trial of Tirzepatide in UK Primary Care Settings
核心洞察
Eli Lilly has initiated the SURMOUNT-REAL UK trial, a five-year randomized study evaluating tirzepatide's long-term effectiveness in primary care settings with up to 3,000 participants in Greater Manchester.
The trial will assess tirzepatide's impact on obesity (搜索) outcomes, type 2 diabetes (搜索) prevention, and broader healthcare measures including quality of life, work productivity, and hospital visits.
Concurrently, Lilly announced price reductions for tirzepatide single-dose vials in the US through its LillyDirect platform to improve access to obesity (搜索) treatments.
Eli Lilly and Company announced on December 1, 2025, that the first patient visit for the SURMOUNT-REAL UK trial has taken place, marking the beginning of a comprehensive five-year study evaluating tirzepatide's real-world effectiveness in treating obesity (搜索). The randomized clinical trial is expected to include up to 3,000 participants in the Greater Manchester region and will assess the drug's long-term impact when administered in primary care settings.
Trial Design and Objectives
The SURMOUNT-REAL UK study represents a significant real-world evidence initiative that will evaluate tirzepatide's effectiveness on obesity (搜索) outcomes while examining its potential to delay the progression of type 2 diabetes (搜索). Beyond traditional clinical endpoints, the trial will assess broader health and healthcare impacts, including quality of life measures, work productivity, employment status, and healthcare utilization patterns such as GP, hospital, and emergency department visits.
"Obesity (搜索) is a pressing health challenge both in the UK and globally," said Professor Rachel Batterham, Senior Vice President for International Medical Affairs at Lilly. "Lilly is committed to working with partners across the health system to better understand and address its complexity. With SURMOUNT-REAL UK, we aim to generate real-world evidence on the long-term health impacts of obesity management medications—evidence that can help shape future care pathways and improve health outcomes for people living with obesity."
Regional Impact and Healthcare Burden
The Greater Manchester region faces a substantial obesity (搜索) burden, with an estimated 600,000 adults living with obesity, representing approximately 27% of the total population. Many of these individuals also suffer from obesity-related illnesses, which reduces their quality of life and places additional pressure on health and care systems. A study by Frontier Economics estimated that the cost of obesity to the Greater Manchester area is around £3.21 billion, encompassing NHS treatment, social care, reduced quality of life, informal care, and impact on economic activity.
Collaborative Research Infrastructure
The trial is being conducted through a partnership between Lilly, Health Innovation Manchester (搜索), and the University of Manchester. North West EHealth (搜索) has been appointed to lead trial delivery, managing data flows, technology, and patient recruitment. Martin Rutter, Professor of Cardiometabolic Medicine at the University of Manchester, serves as the Chief Investigator.
"In addition to endpoints assessing weight reduction and progression to type 2 diabetes (搜索), SURMOUNT-REAL UK will assess the broader health and healthcare impacts in a diverse group of individuals living across Greater Manchester," Rutter stated. "The trial is particularly important because it will contribute to the evidence base on intervening comparatively early in the progression of obesity (搜索)."
Primary Care Integration and Health Equity
The study emphasizes the importance of conducting research in primary care settings to address health inequalities and ensure inclusive participation. Dr. Imran Ghafoor, GP Partner at Peterloo Medical Centre and Principal Investigator for the trial, highlighted the significance of community-based research.
"As a GP, I see patients bring more than symptoms—they bring hopes, challenges, and the realities of their communities. They trust our practice as a familiar and accessible space," Ghafoor noted. "This is why I am proud to be involved in this trial, which extends participation opportunities to groups often under-represented in research. Research in primary care gives the potential to address health inequalities we witness firsthand."
US Market Access Improvements
Simultaneously with the UK trial announcement, Lilly revealed price reductions for tirzepatide single-dose vials in the United States, where the drug is marketed as Zepbound. The company is offering reduced pricing through its digital healthcare platform, LillyDirect, following similar price reductions for multi-dose pens in November.
"Far too many people who need obesity (搜索) treatments still face cost and coverage barriers," said Ilya Yuffa, executive vice president and president, Lilly USA and Global Customer Capabilities. "Today's action underscores Lilly's commitment to improving access across the obesity care landscape. We will keep working to provide more options—expanding choices for delivery devices and creating new pathways for access—so more people can get the medicines they need."
Current Treatment Landscape
Tirzepatide, a dual GLP-1 (搜索)/GIP (搜索) agonist marketed under the Mounjaro trade name, has been recommended for NHS use in obese individuals with a BMI of 35 or more and at least one weight-related illness. The drug is also indicated for people with type 2 diabetes (搜索) when standard therapies prove ineffective at controlling blood glucose levels.
The SURMOUNT-REAL UK trial results will be published in peer-reviewed journals and are expected to provide crucial evidence for policy formulation regarding the integration of weight-loss drugs into the NHS and the overall management of obesity (搜索) in the UK healthcare system.
