Long-term Study Reveals Similar Outcomes for ACS Patients With and Without Traditional Risk Factors

A large-scale multicenter study from China has revealed that acute coronary syndrome (search) (ACS) patients without traditional cardiovascular risk factors face similar long-term outcomes as those with established risk factors, challenging conventional risk assessment approaches.
The study, analyzing data from the Optimal antiPlatelet Therapy for Chinese patients with Coronary Artery Disease (search) (OPT-CAD (search)) registry, examined 5,688 ACS patients who underwent percutaneous coronary intervention (PCI). Among these, 392 patients (6.9%) had no standard modifiable cardiovascular risk factors (SMuRFs), which include hypertension (search), diabetes (search), dyslipidemia (search), and smoking.
Comparable Long-term Outcomes
The research team found no significant differences in 5-year major adverse cardiac-cerebrovascular events (MACCE) between patients with and without SMuRFs. The MACCE rates were 9.44% for SMuRFs-absent patients compared to 9.76% for those with risk factors (log-rank P = 0.90).
"These findings challenge our traditional understanding of cardiovascular risk assessment," notes the study team. "The absence of conventional risk factors does not necessarily indicate a better prognosis."
Key Risk Factors Identified
The study identified two independent predictors of adverse outcomes in SMuRFs-absent patients:
- Age (HR: 1.06; 95% CI: 1.03–1.10; P = 0.001)
- Presence of thrombus lesions (HR: 2.58; 95% CI: 1.24–5.40; P = 0.011)
Treatment Patterns and Clinical Implications
Analysis of medication patterns revealed that SMuRFs-absent patients received different treatment approaches:
- Lower utilization of β-blockers (71.68% vs 76.44%)
- Reduced prescription of ACE inhibitors or ARBs (53.83% vs 71.24%)
- Higher use of statins (98.98% vs 96.60%)
Future Directions
The findings highlight the need for updated risk assessment models and new biomarkers to identify at-risk patients without traditional risk factors. Researchers suggest investigating novel factors such as clonal hematopoiesis of indeterminate potential (CHIP) as potential prognostic markers.
A multicenter, prospective observational registry (ACTRN12622000452796) has been established to further study and improve the clinical pathway for SMuRFs-absent coronary artery disease (search) patients.
