The University of Minnesota Medical School study demonstrates that the widely used diabetes medication significantly reduces the risk of developing Long COVID when taken during or shortly after acute SARS-CoV-2 (search) infection.
Breakthrough Prevention Strategy
The comprehensive analysis summarized several studies across low, standard, and high-risk populations, revealing that taking metformin during or soon after SARS-CoV-2 (search) infection prevents the development of Long COVID (search). The research team calculated the number needed to treat (NNT) for each study, finding that on average across four studies, one case of Long COVID is prevented for every 50 cases of acute SARS-CoV-2 treated with 14 days of metformin.
"Metformin has emerged as the first intervention to prevent Long COVID (search) in low to high risk adults and across multiple clinical trials and high-quality electronic health record studies," said Carolyn Bramante, MD, MPH, an assistant professor at the University of Minnesota Medical School and physician with M Health Fairview (search). "This body of evidence means that starting metformin at the time of infection can reduce the risk of Long COVID for most adults who get COVID-19 (search) today."
Clinical Trial Evidence
The research represents the first intervention tested in two large, placebo-controlled, randomized trials specifically designed to prevent Long COVID (search) across diverse adult populations, including those with prior infections. Notably, two of the research studies included participants who were also receiving FDA-approved treatments for SARS-CoV-2 (search), including nirmatrelvir-ritonavir (search), molnupiravir, and remdesivir, demonstrating metformin's compatibility with existing therapeutic protocols.
Additional Therapeutic Benefits
Beyond Long COVID (search) prevention, the studies revealed that metformin provides additional antiviral benefits. Compared to placebo, metformin significantly decreases SARS-CoV-2 (search) viral load and prevents rebound of viral load during acute infection. The medication demonstrated a strong safety profile during acute COVID-19 (search), with no reports of low blood sugar or serious side effects.
Clinical Implementation
The research team emphasized that metformin can be used alone or combined with other treatments for acute SARS-CoV-2 (search) infection. Most adults can take metformin, though patients should consult their clinician before starting treatment. The findings apply to most adults being infected with COVID-19 (search) in the current pandemic state.
Study Limitations and Future Research
The research identified important knowledge gaps that require further investigation. It remains unknown whether metformin can prevent Long COVID (search) in pediatric populations, and the studies did not examine whether metformin can treat existing Long COVID in either adults or children. These limitations highlight the need for additional research to expand the therapeutic applications of metformin in COVID-19 (search) management.
