Survey Study Reveals Dramatic Risk Reduction
A comprehensive survey-based study published June 27 in the American Journal of Obstetrics and Gynecology examined more than 5,400 participants recruited through the Hyperemesis Gravidarum Education and Research Foundation (search)'s social media platforms. Participants completed online questionnaires reporting their daily use of 32 common substances, including prescribed medications, in the month before each pregnancy, along with their pregnancy nausea and vomiting levels.

Cohort Study Provides Additional Evidence
A separate observational cohort study analyzed data from 80 women exposed to metformin before conception and 4,411 non-exposed women enrolled in MotherToBaby Pregnancy Studies (search) between 2012 and 2023. While the study wasn't powered to detect statistical significance, it found a lower frequency of HG among metformin-exposed women (1.25%) compared to unexposed women (2.20%), with an adjusted risk ratio of 0.50.

Study Limitations and Next Steps
While promising, the findings come with important caveats. The survey study relied on self-reported data from a highly selective population, as noted by Dr. Stephen O'Rahilly, a professor of clinical biochemistry and medicine at the University of Cambridge who was not involved in the research. He emphasized that clinical trials are now needed to confirm whether prescribing metformin before pregnancy could effectively prevent hyperemesis.
Future clinical trials will need to address several key questions, including optimal metformin dosing and timing of administration. The studies didn't examine the drug's effect on morning sickness (search) when started during pregnancy or track how many participants discontinued the medication after conception.
Metformin is considered safe during pregnancy, and many users continue taking it throughout pregnancy to manage diabetes. However, for individuals not already taking metformin before pregnancy, the medication might come too late to prevent hyperemesis in the current pregnancy, though it could theoretically prevent recurrence in subsequent pregnancies.
Brecht-Doscher concluded that "this study clears the way for clinical trials, because we've shown it's worth pursuing." The research represents a significant step toward understanding and potentially preventing a condition that has long lacked effective prevention strategies.
