MHRA Strengthens Safety Warnings for Botulinum Toxin Type A Products Following Rare Cases of Iatrogenic Botulism

The updated warnings, which will appear in the Summary of Product Characteristics (SmPC) and Patient Information Leaflet (PIL) for all UK-authorised products in the coming months, aim to more clearly highlight the risk of iatrogenic botulism (search) and advise patients to seek immediate medical attention if symptoms arise.
Dr Alison Cave, MHRA Chief Safety Officer, stated: "Botulinum toxin products are effective for a number of different medical and cosmetic purposes when used correctly. However, these strengthened warnings are an important reminder that, in rare cases, serious side effects can occur."
"It is vital that healthcare professionals and patients are aware of the symptoms of botulism and act quickly if they arise as this is a medical emergency. We also strongly urge the public to avoid unlicensed products and seek treatment only from appropriately qualified practitioners," Dr Cave added.
Clinical Presentation and Risk Factors
Iatrogenic botulism (search) occurs when the toxin's effect extends beyond the area of treatment. Symptoms can take up to four weeks to develop and may typically include difficulty breathing or shortness of breath, difficulty swallowing, talking or slurred speech, and muscle weakness. In severe cases, patients may require mechanical ventilation and intensive care treatment.
The MHRA has identified several circumstances that may increase the risk of adverse reactions from the spread of toxin or botulism. These include patients with underlying conditions and comorbidities such as neurological disorders or a history of dysphagia or aspiration; use of high doses of botulinum toxin; off-label use with administration into unapproved sites; and use of counterfeit or unauthorised botulinum toxin products.
Clinical Management Guidance
Healthcare professionals managing patients with suspected botulism should follow UK Health Security Agency (UKHSA (search)) advice on clinical management. Treatment can vary and may include the use of botulinum antitoxin to neutralise circulating toxin, supportive therapy, and treatment of specific symptoms. Recovery depends on the severity of symptoms.
In patients presenting with only localised side effects where administration of botulinum antitoxin is not indicated, monitoring for further progression of symptoms and presentation of systemic neurological symptoms may be necessary.

There are effective treatments available and we recommend seeking immediate medical advice if you have had a recent treatment and are having symptoms such as difficulty swallowing."
Regulatory Context and Unlicensed Products
Botulinum toxins are prescription-only medicines in the UK and should only be sold or supplied in accordance with a prescription given by an appropriate prescriber. Licensed indications vary by product and include neurologic disorders such as focal spasticity, certain types of spasms, chronic migraine, bladder disorders including overactive bladder, severe axillary hyperhidrosis, and temporary improvement in the appearance of facial lines where their severity has an important psychological effect on the patient.
The MHRA has emphasised that units of botulinum toxin are not interchangeable from one product to another, and the product information contains specific recommendations for each authorised product including indications, administration sites, and dosing guidance.
The MHRA's Criminal Enforcement Unit has launched a number of investigations linked to the use of unlicensed botulinum toxin products following a spike in hospital admissions seen last year. The agency warns that buying botulinum toxins from illegal suppliers significantly increases the risk of obtaining products that are either falsified or not licensed for use in the UK, meaning there are no safeguards to ensure products meet MHRA standards for quality and safety.
Reporting and Monitoring
Serious adverse reactions due to the distant spread of toxin have been closely monitored and reflected in product information since 2007. The MHRA continues to monitor the safety of these medicines, including side effects and emerging evidence. Healthcare professionals, patients, and caregivers are asked to submit reports using the Yellow Card scheme electronically via the Yellow Card website, the Yellow Card app, or through certain clinical IT systems. When reporting suspected adverse drug reactions, reporters should provide as much information as possible, including medical history, concomitant medications, onset timing, and treatment dates.
