Lynn Sutfin, a spokesperson for the Michigan Department of Health and Human Services (搜索) (MDHHS), confirmed the case count as of 4:30 p.m. on July 1, noting that affected individuals range in age from 8 to 84. The surge is dramatically out of proportion to historical norms: "Michigan typically identifies 50 cases a year," an MDHHS spokesperson told TODAY.com.
The U.S. Centers for Disease Control and Prevention (搜索) (CDC) and the Michigan Department of Agriculture and Rural Development (搜索) (MDARD) have joined state and local health officials in the investigation. A statewide health alert was issued June 30, directing physicians and medical providers to test stool samples from patients presenting with diarrhea and other symptoms consistent with cyclosporiasis (搜索), and to report positive results to local health departments within 24 hours.
"We have identified an unusual number of cases in a little over a week, and we anticipate additional cases of illness being reported," said Dr. Natasha Bagdasarian, chief medical executive at MDHHS. "We recommend Michiganders contact their health care provider if they experience sudden, ongoing diarrhea and reach out to their local health department if additional members of their family are suffering from the same symptoms."
Cross-Border Concerns in Ohio
The outbreak may extend beyond Michigan's borders. The Toledo-Lucas County Health Department (搜索) in northwest Ohio is investigating 65 cases of cyclosporiasis (搜索) identified between June 25 and 7 a.m. on July 1, according to department spokesperson Sarah Velliquette.
Health officials in Ohio are collaborating with "partners across the region as part of this ongoing investigation by sharing information and reviewing potential commonalities," Velliquette said, noting coordination with health departments in southeastern Michigan, the Ohio Department of Health, the Ohio Department of Agriculture, and area hospitals.
However, the CDC has not confirmed any linkage. In an emailed statement, CDC Press Officer Lisa George said: "CDC has no evidence of a single, multistate Cyclospora outbreak linking cases happening right now and being reported in the press. The data currently found on CDC's website is a surveillance count of cases across the United States of people with cyclosporiasis (搜索). This number includes clusters of cases currently under investigation by the FDA and cases that have not been linked to a common source."
As of July 1, the CDC surveillance website listed only 145 cases nationally identified through June 16 — a figure that predates both Michigan's first reported case on June 22 and the 65 cases in Lucas County, Ohio, reported after June 25.
Disease Characteristics and Transmission
Cyclosporiasis (搜索) is caused by Cyclospora cayetanensis (搜索), a microscopic parasite common in tropical and subtropical regions. The infection spreads through the fecal-oral route when people consume food or water contaminated with feces containing the parasite.
According to the CDC, it typically takes one to two weeks outside the body for the parasite to become infectious, making human-to-human transmission highly unlikely. Previous U.S. outbreaks have frequently been linked to imported fresh produce, including basil, cilantro, mesclun lettuce, raspberries, and snow peas.
Symptoms typically begin two to 14 days after exposure and include watery, explosive diarrhea, abdominal cramps and bloating, loss of appetite, weight loss, nausea, and low-grade fever. Vomiting is less common but has been reported. The illness can persist for days to over a month if untreated, and symptoms may relapse after initially resolving.
National Context
Nationwide, at least 145 cases of cyclosporiasis (搜索) were reported across 17 states between May 1 and June 16, according to CDC data that does not yet reflect the Michigan surge. New York has reported the highest number of cases, followed by Illinois and Texas. The infected range in age from 5 to 86 years old, with at least 20 hospitalizations and no deaths reported.
Cyclosporiasis (搜索) is a seasonal illness in the U.S., with cases typically peaking between May and August. In recent years, reported infections have increased significantly: an unusually high 4,703 cases were identified in 2019, according to a 2023 CDC report.
Investigation and Prevention
MDARD spokesperson Jennifer Holton said the agency is in the initial phases of its investigation, working to construct an exposure timeline by examining the food history of each infected individual. "There is not a point source identified at this time," Holton said. "Once a product of interest is identified, we will continue to work with our investigative partners and/or lead the formal trace-back process and begin to examine the supply chain."
In the interim, health authorities recommend washing hands with soap and water before and after handling raw produce, thoroughly washing all fruits and vegetables under running water, scrubbing firm produce with a clean brush, cutting away damaged areas, and refrigerating cut or peeled produce within two hours.
The infection is treatable with antibiotics. Individuals with healthy immune systems may recover without treatment, though those with weakened immune systems face higher risk for prolonged illness and symptom relapse.
County-Level Case Distribution
As of July 1, the known case distribution across Michigan includes: Monroe (92), Lenawee (36), Washtenaw (28), Wayne excluding Detroit (17), Jackson (11), Shiawassee (9), Ingham (6), Oakland (5), Detroit (3), Livingston (3), Sanilac (3), Hillsdale (2), Lapeer (2), and one case each in Eaton, Genesee, Gratiot, Houghton, Leelanau, Manistee, Ottawa, and Saginaw counties. State authorities caution that some cases may be unrelated to the current outbreak.
