The trial, conducted in 157 patients with advanced melanoma (搜索) who had undergone surgery to remove their tumours, tested the combination of an mRNA vaccine with immunotherapy (搜索) over one year of treatment. This represents the first major clinical validation of mRNA technology's potential in cancer (搜索) therapeutics, building on the success of COVID-19 vaccines developed by companies like Moderna, Pfizer, and BioNTech.
Personalized Medicine Approach
Unlike conventional vaccines that prevent disease, these mRNA cancer (搜索) vaccines function as personalized treatments that train each patient's immune system to fight their specific cancer. The process begins with taking samples of the patient's tumour and healthy tissue, followed by DNA sequencing to identify problem mutations that can serve as vaccine targets.
"The genomics in the tumour cell are chaotic," explains Prof Alan Melcher from the Institute of Cancer (搜索) Research. "There's stuff turning into protein that shouldn't be turning into protein and there are other places where big chunks of DNA just either get dropped out or inserted or turned around."
The personalized approach allows scientists to target multiple cancer antigens (搜索) simultaneously, decreasing the likelihood that cancer (搜索) cells will develop resistance. According to the research, it takes approximately two months to produce an individualized vaccine for each patient.
Expanding Clinical Pipeline
Following the promising melanoma (搜索) results, Moderna and MSD plan to initiate a phase III trial for advanced melanoma in 2023. If successful, they could apply for regulatory approval within the next couple of years. The US FDA has already granted breakthrough therapy designation to the vaccine, allowing expedited development and review.
BioNTech, the German company that partnered with Pfizer for COVID-19 vaccines, has multiple mRNA cancer (搜索) candidates in development, including treatments for advanced melanoma (搜索), ovarian cancer (搜索), and non-small cell lung cancer (搜索). The company expects to release results from its own phase 2 melanoma trial of 131 patients later this year, measuring cancer progression and survival over 24 months.
CureVac, the third major player in mRNA cancer (搜索) vaccines, is targeting cancers ranging from ovarian to head and neck, colorectal, lung, and pancreatic. The company has struck an innovative deal with Tesla (搜索) to develop small, portable mRNA bioprinters that could automate vaccine production and be shipped to remote locations.
Clinical Applications and Future Prospects
BioNTech's co-founder Uğur Şahin foresees two main applications for mRNA cancer (搜索) vaccines. The first involves combination approaches with CAR T-cell or other cell therapies to shrink large, rapidly growing tumours in patients with advanced disease. The second targets patients who have recently undergone surgery to prevent cancer recurrence and metastasis.
"As an example in colorectal cancer (搜索) patients, about 30-40% of patients have a relapse after surgery in the first three years," Şahin explains. "But we can give these patients a circulating tumour DNA test, which tells us if there are still cancer (搜索) cells lingering after the operation and, if it's positive, those patients will receive the vaccine."
Healthcare System Integration
The NHS has launched a groundbreaking partnership with BioNTech to fast-track mRNA cancer vaccine (搜索) development over the next seven years. Eligible cancer (搜索) patients in the UK will gain early access to clinical trials from autumn 2023, with the goal of making these treatments clinically available to around 10,000 cancer patients by 2030.
However, the cost of personalized medicines remains a significant challenge. Christopher Scott, a cancer (搜索) research professor at Queen's University Belfast, notes concerns about NHS delivery capabilities: "I remain unconvinced about whether an entirely personalised vaccine approach could be delivered in our NHS. Because of the Covid vaccines they've now got manufacturing processes that have been passed by regulators, which is fantastic, but this is still an expensive technology."
Manufacturing and Cost Considerations
Companies are implementing strategies to reduce production costs as manufacturing scales up. Şahin believes costs can decrease significantly with volume: "If you produce personalised vaccines for 1,000 patients per year, it's a completely different equation compared with producing them for 10,000 or 100,000 patients per year."
An alternative approach involves developing off-the-shelf mRNA cancer (搜索) vaccines using standard antigens common across multiple patients, particularly for slower-progressing cancers where highly personalized targeting may be less critical.
Technological Challenges
Despite the promising results, researchers acknowledge significant hurdles remain. The primary challenge involves identifying the most relevant mutations that truly drive cancer (搜索) progression. Norbert Pardi, an assistant professor at the University of Pennsylvania, has observed trials where vaccines stimulated immune responses without significant tumor impact.
"I think this is the most important hurdle that we need to overcome," Pardi states. "Why don't we always see benefit in patients even in the presence of a robust immune response?"
With rapidly falling genome sequencing costs—the world's first $100 genome was announced last year—companies are exploring whole-genome sequencing to identify subtler targets. CureVac is pioneering this approach, focusing on structural variations that previous efforts have ignored.
Şahin remains optimistic about the timeline for widespread availability: "We believe that this will happen on an even broader scale before 2030. The Covid-19 vaccine and our expertise in developing it has contributed to our work in oncology."
