Natera's Signatera MRD Test Shows Powerful Prognostic Value in Phase III PALLAS Breast Cancer Study

Natera Inc. has unveiled compelling translational research results from the international randomized Phase III PALLAS study, demonstrating the powerful prognostic value of molecular residual disease (MRD) testing using its Signatera (search) ctDNA (search) test in stage II-III HR+/HER2- breast cancer (search) patients. The findings, presented at the San Antonio Breast Cancer (search) Symposium, support a fundamental shift toward MRD-informed personalized cancer care.
Strong Risk Stratification Across Treatment Timeline
The analysis of 420 patients from the U.S. biomarker cohort revealed that Signatera (search) MRD status consistently predicted distant recurrence risk across three critical postoperative timepoints: baseline (first day of protocol-directed therapy), approximately six months on-treatment (C6D1), and at the end of the two-year treatment period (EOT).
At baseline, approximately 92% of patients tested MRD-negative and demonstrated excellent outcomes with a five-year distant recurrence-free interval (DRFI) of 93%. This favorable prognosis improved further by end of treatment, with MRD-negative patients achieving a five-year DRFI of 95%.
In stark contrast, the approximately 8% of patients who tested MRD-positive at baseline faced dramatically different outcomes, with a five-year DRFI of just 28%. At end of treatment, MRD-positive patients maintained poor prognosis with a five-year DRFI of 32%.
Unprecedented Risk Separation
The study revealed hazard ratios that far exceed typical clinicopathologic risk factors. MRD-positive patients demonstrated hazard ratios ranging from 13.4 to 21.5 compared with MRD-negative patients across all timepoints, showing much larger risk separation than is typically seen with individual clinicopathologic factors alone.

"Implementation of longitudinal post-surgical ctDNA (search) testing with Signatera (search) advances us beyond a one-size-fits-all management approach based solely on initial tumor characteristics," explained Minetta Liu, M.D., chief medical officer of oncology and early cancer detection at Natera. "Instead, we can start to imagine treatment algorithms where ctDNA-negative patients are spared unnecessary treatment with related toxicity, and ctDNA-positive patients are prioritized for more intensive or novel therapies."
About the PALLAS Trial Context
The PALLAS (PALbociclib CoLlaborative Adjuvant Study) trial was a randomized, prospective, international, multicenter Phase 3 study comparing palbociclib in combination with endocrine therapy versus endocrine therapy alone for adults with HR+, HER2- Stage II and Stage III early breast cancer (search). While the primary trial did not meet its endpoint for palbociclib efficacy, the predefined MRD biomarker analysis has provided valuable insights into risk stratification.
The translational research results represent the first dataset from the U.S. cohort, with data from a parallel ex-U.S. cohort and additional subgroup analyses planned for future presentation.
