Nerivio Wearable Device Shows No Tachyphylaxis in 3-Year Migraine Study

A prospective real-world study spanning three years has demonstrated that Theranica (搜索)'s Nerivio (搜索) remote electrical neuromodulation (REN) wearable device maintains consistent migraine (搜索) treatment effectiveness without developing tachyphylaxis, providing new evidence for long-term drug-free migraine management.
The study, published in Headache, analyzed data from 224 patients with migraine (搜索) in the United States who used the REN wearable device monthly from December 2019 to September 2024. Participants included individuals aged 12 years and older (mean age 43.8 ± 16.2 years; 85.7% women and girls) who received treatment with 45-minute sessions at least once monthly for nine months out of each year.
Sustained Efficacy Without Tolerance Development
The primary endpoint focused on the absence of tachyphylaxis, defined as increases of no more than 2.5 intensity units on a 100-unit scale between consecutive years. Results showed no clinically meaningful change in treatment intensity over the three-year period, with average changes between consecutive years remaining well below the threshold (1.8 [±5.5] between years 1 and 2, and 1.4 [±5.3] between years 2 and 3; P = 0.120).
Treatment intensity increased by only a median total of 3.2 units from year one to year three (35.0 ± 16.2 in year 1; 36.8 ± 17 in year 2; 38.2 ± 18.3 in year 3), indicating patients did not require higher stimulation intensities or treatment adjustments to maintain therapeutic benefits.
Consistent Treatment Outcomes Across Patient Populations
Both youth and adult participants maintained stable therapeutic outcomes throughout the study period. Pain relief was consistently reported by 72.1% to 76.8% of users across all three years, while functional disability relief was reported by 65.3% to 70.8% of patients. Additional efficacy measures remained stable, including pain freedom (26.8% to 28.7%), functional disability freedom (31.4% to 38.9%), and freedom from associated symptoms such as photophobia (搜索) (29% to 37%) and phonophobia (搜索) (37.9% to 49.4%).
"The publication of this multi-year, month-by-month, real-world evidence study in Headache marks a watershed moment in migraine (搜索) care," said Alit Stark-Inbar, PhD, corporate vice president of medical information and research at Theranica (搜索). "The results show that REN therapy provides consistent, long-term effectiveness without the need for a dosage increase over time, alongside sustained adherence to therapy."
Safety Profile and Patient Adherence
The device demonstrated excellent tolerability with only two nonserious device-related adverse events reported—mild or moderate skin reactions—neither of which resulted in treatment discontinuation. Patient adherence remained consistent throughout the study, with utilization ranging between 8 and 8.8 treatments per month over the three-year period.
Clinical Context and Treatment Timing
The Nerivio (搜索) REN wearable operates as both an acute and preventive prescription migraine (搜索) treatment using electrical pulses applied to the arm to activate the brain's natural pain regulation system. The device is controlled via smartphone app and works without drugs, needles, or invasive procedures.
Previous research has shown that early treatment initiation enhances the device's efficacy. A large-scale real-world evidence study demonstrated that initiating Nerivio (搜索) treatment within one hour of migraine (搜索) onset can improve efficacy outcomes by up to 2-fold compared with later treatment initiation, with early treatment yielding significantly higher responder rates (P < .001) across multiple migraine symptoms.
"Individuals with migraine (搜索) often struggle to find long-term relief due to medication side effects, drug–drug interactions, or reduced effectiveness over time," noted Stephanie J. Nahas, MD, director of the headache medicine fellowship program at Thomas Jefferson University Hospital and lead author of the study. "This supports previous findings, suggesting that REN offers an effective, well-tolerated, safe, and sustainable long-term drug-free treatment option for a wide range of individuals with migraine."
The findings are particularly relevant given that more than one-third of Nerivio (搜索) users are younger than 18 years when initiating treatment, addressing a critical need in adolescent migraine (搜索) management where traditional pharmacological approaches may be limited by safety concerns and long-term effects.
